Full Job Description

Qualifications
Master’s Degree in Social Work (MSW)

Job description:

Conducts home visits, intake assessments, scheduling, evaluations, case management, crisis intervention and follow- up services for Home Sharing and Respite Care Program.
Handles intakes for new clients for the Home Sharing and Respite Care Program
Schedule guest and host appointments
Participates in the service delivery components of Home Sharing Program, follow-up, outreach, entitlements and benefits assistance, advocacy, etc.
Assists with assessment and matching of clients with extensive follow-up
Prepares and organizes case files
Answer program related telephone inquiries and provides information and referral services
Develops and maintains ongoing outreach with legislators, local media, community resources
Makes follow up phone calls to all Home Sharing clients after a match is made
Assists Home Sharing and Respite Care Program Director with record keeping and in insuring timely preparation of all required reports
Verifies all references for Home Sharing and Respite Care applicants
Assists with follow up to physicians regarding Respite Care Program medical form
Other tasks as assigned
