Full Job Description

Job Summary

We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team for the evening shift, working Monday through Friday from 2pm to 10pm. In this vital role, you will provide compassionate patient care, support clinical operations, and ensure the well-being of our residents and clients. Your expertise will help create a safe, comfortable environment while delivering high-quality nursing services in a dynamic healthcare setting. Responsibilities

Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
Monitor patients’ conditions closely, managing acute pain and providing appropriate interventions in accordance with long-term care regulations and best practices.
Assist with specialized care procedures such as tube feeding, airway management, and sterile processing techniques.
Document all patient interactions thoroughly, ensuring compliance with HIPAA and clinical confidentiality policies.
Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents with dementia, Alzheimer’s disease, or other memory care needs.
Support emergency response efforts by applying first aid skills, CPR certification, and trauma center experience when necessary.

Requirements

Valid LVN license in good standing with the state licensing board.
Proven experience working in diverse healthcare environments such as skilled nursing facilities, hospitals (including ICU or trauma centers), assisted living communities, or home care settings.
Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with ICD-9/10 coding is a plus.
Knowledge of geriatrics, dementia care, hospice care, infectious disease management, and acute care procedures.
Ability to perform physical examinations and assessments accurately while maintaining patient dignity and confidentiality.
Strong communication skills to effectively collaborate with patients, families, and healthcare teams.
Certification in Basic Life Support (BLS) and First Aid; additional certifications such as IV insertion or phlebotomy are advantageous.
