Full Job Description

At the Institute on Aging, we believe growing older shouldn't mean losing your independence. As a Care Manager, you'll be the steady, trusted presence that helps vulnerable adults navigate complex medical, social, and emotional challenges, so they can remain safely in the homes and communities they love.

This is meaningful, hands-on work. You'll conduct home visits, coordinate with medical teams, and build real relationships with clients and their support networks, all while drawing on your clinical training to make thoughtful, person-centered care decisions every day.

What You'll Do

Conduct comprehensive assessments of clients' psychosocial, physical, mental health, environmental, and spiritual needs, and develop individualized Community Living Plans in partnership with each client
Make home visits, hospital and skilled nursing facility visits, and accompany clients to medical appointments as needed
Provide clinical oversight and support to fellow Care Managers and Community Health Workers
Connect clients to the right community resources by drawing on your knowledge of Medicare, Medi-Cal, and related entitlement programs
Offer crisis intervention, advocacy, and therapeutic support, meeting with clients at least monthly (more often when needed)
Maintain clear, timely documentation and collaborate closely with physicians, providers, and your care team
Continuously evaluate whether clients can safely remain at home, and coordinate alternative care when circumstances change

What You Bring

A Master's degree in Social Work (MSW) or another master's-level, licensure-eligible field (MFT, Counseling, Psychology, or similar)
At least one year of experience working with older adults and/or adults with disabilities
A solid understanding of the medical and psychosocial challenges facing functionally impaired and older adults
Experience with mental health, behavioral health, substance use, or unhoused populations is a strong plus
Excellent communication skills, strong attention to detail, and the ability to juggle a busy, varied caseload
Comfort with computers and electronic medical records; EPIC experience is a plus
