Full Job Description

At Muncie Primary Care, we believe healthcare doesn't stop when a patient leaves the office.

We're looking for a compassionate Licensed Practical Nurse (LPN) to join our Care Management team. This position is ideal for someone who enjoys building relationships with patients, providing education, and helping people successfully manage chronic conditions. (_We're open to hiring one full-time nurse or two part-time nurses. We'd love to find the right fit for our team. Full time is 40 hr/week, part time is less than 29 hr/week_)

Unlike a traditional clinic nursing role, this position focuses on proactive patient care between office visits. You'll work closely with providers and our Care Management team to help patients improve their health, stay on track with treatment plans, and avoid unnecessary emergency room visits and hospitalizations.

What You'll Do

Provide telephone support for patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
Review patient information within our care management platform.
Monitor stable INR and Continuous Glucose Monitor (CGM) results using established clinical protocols.
Complete routine patient follow-up calls and wellness check-ins.
Educate patients on chronic disease management and reinforce provider treatment plans.
Update care plans and document patient interactions in the electronic health record.
Identify patients who need additional clinical attention and communicate concerns to the Care Management RN or provider.
Assist with enrolling eligible patients into care management programs.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.

Qualifications

Required

Current Indiana Licensed Practical Nurse (LPN) license in good standing.
Excellent communication and patient education skills.
Strong organizational and documentation skills.
Comfortable using electronic health records and computer systems.
Ability to work independently while collaborating with a healthcare team.

Preferred

Experience in primary care, home health, care management, case management, family medicine, geriatrics, or chronic disease management.
Experience working with Medicare patients is a plus.
