Overview
We are seeking a dedicated and detail-oriented MDS (Minimum Data Set) Coordinator to join our dynamic healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of patient care in skilled nursing facilities, ensuring compliance with federal and state regulations. Your expertise will support optimal patient outcomes, accurate clinical documentation, and seamless communication across multidisciplinary teams. This position offers an exciting opportunity to influence quality care delivery while working within a collaborative, fast-paced environment committed to excellence.

Duties

Lead the completion and accuracy of MDS assessments, ensuring they reflect current patient conditions and care plans in accordance with CMS (Centers for Medicare & Medicaid Services) standards.
Collaborate with nursing staff, physicians, social workers, and other healthcare professionals to gather comprehensive clinical information for each resident.
Review medical records, clinical documentation, and coding details including ICD-10/ICD-9/ICD coding, CPT coding, and DRG (Diagnosis-Related Group) assignments to support proper reimbursement and compliance.
Ensure timely submission of MDS assessments within regulatory deadlines while maintaining high-quality standards aligned with NCQA (National Committee for Quality Assurance) standards.
Conduct utilization review and management activities to optimize patient care pathways, discharge planning, hospice care coordination, and transition processes such as home care or outpatient services.
Utilize EMR (Electronic Medical Records) systems such as Cerner, Epic, Athenahealth, or eClinicalWorks to document assessments accurately and efficiently.
Maintain strict adherence to HIPAA regulations and ensure all medical documentation is complete, accurate, and compliant with privacy standards.

Qualifications

Proven experience in MDS coordination within a skilled nursing facility or long-term care setting; experience in hospital or rehabilitation environments is highly valued.
Strong knowledge of Medicare regulations, NCQA standards, and federal guidelines related to skilled nursing facilities.
Familiarity with EMR/EHR systems including Cerner, Epic, Athenahealth, or similar platforms.
Expertise in ICD-10/ICD-9/ICD coding, CPT coding, DRG assignment, and utilization management processes.
Critical care experience or ICU background is preferred; experience with discharge planning in acute or post-acute settings is a plus.
Excellent understanding of medical terminology, anatomy physiology knowledge, and clinical documentation improvement strategies.
Ability to review complex medical records accurately while maintaining attention to detail under tight deadlines.
Strong communication skills for effective collaboration with multidisciplinary teams and external providers.
Nursing licensure or relevant healthcare certifications are preferred; familiarity with managed care environments is advantageous.
