Full Job Description

POSITION SUMMARY

The PSR Counselor will assist members by helping to solve a variety of life-skill challenges through facilitation of classes that revolve around mental health, substance abuse, independent living, social skills, job readiness, literacy, and member advocacy. This position will require facilitating classes, and individual meetings, as well as setting individualized goals and objectives to help members with mental health and substance abuse disorders.

Supervisory Responsibilities: N/A

ESSENTIAL DUTIES AND RESPONSIBILITIES: PSR Counselor will meet with members on a regular basis and duties may include, but are not limited to the following:

· Drive the agency van to pick up / drop off members whose transportation to the center is provided by the agency.

· Drive agency van to attend field trips.

· Establish and maintain a peer relationship rather than a hierarchical relationship with members.

· Conduct intakes and new member orientations as it pertains to the program.

· Monitor and document member progress in the program and shares notes with team members.

· Assist in providing activities, events, and experiences for members’ successful step-work to led groups and/or individual encounters.

· Teach, model, and coach member in skills needed to maintain a healthy life in (independent living, employment, housing, self-care)

· Teach, model and coach members in skills needed for self-management of SUD/mental health symptoms and prevention of relapse.

· Assist in identifying triggers and ways to cope with triggers/urges.

· Developing a personal plan of care and perform other tasks relating to members’ plan of care, as needed.

· Provide accountability/coaching for members, relating to step-work, obtaining a sponsor, attending meetings, assignments, service work, etc.

· Meet with the Program Manager weekly.

· Attending Team Meeting weekly

· Develop curriculum for classes and activities.

· Complete all training requirements (preservice, in-service, developmental, and corrective action).

· Facilitate Mental Health Wellness group and classes.

· Facilitate Substance Abuse group and classes.

· Facilitate Social Skills and independent livings groups and classes.

· Facilitate Employment Support groups and classes.

· Establish and Facilitate volunteer and employment opportunities.

· Plan and conduct scheduled activities.

· Be responsible for a caseload and advocate for individuals assigned to your caseload.

· Assist in the development of WRAP Plans and assist with the completion Declaration of Mental Health Packet.

· Create and amend Discharge Plans.

· Establish and facilitate necessary agency and member focused meetings.

· Create and maintain member binders.

· Assist with meal prep.

· Assist with maintaining the facility and agency property.

· Schedule Transportation for members.

QUALIFICATIONs:

High School diploma or GED required. Bachelor's degree is preferred.

Language skills

Strong oral and written skills are required in this position including.

· Ability to effectively present information to individuals, staff, and the general public including providing training on a variety of mental health topics.

· Ability to respond to inquiries or complaints from customers, agencies, or members of the public.

· Ability to write clear and concise reports and correspondence.

· Ability to read, analyze and interpret professional articles, reports and documents.

· Excellent skills in building and sustaining collaborative relationships.

· Organizational and computer skills

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, common fractions, and decimals. Ability to compute percentages.

Reasoning Ability

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret technical instructions and deal with several abstract and concrete variables.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit, talk, hear, and use hands to write, type or operate office equipment. During a typical day an employee is required to stand and walk. The employee must occasionally lift and/or move up to 10 lbs. Specific vision requirements include close vision, distance vision, color vision and ability to adjust and focus.
