Behavioral Health Clinician

ACMHC, LCMHC, LCSW, LMFT

Location: Primary Care Clinic
Schedule: Full-Time
Clinic Hours: Monday through Friday, 8:00 AM–5:00 PM
Reports To: Medical Director / Behavioral Health Leadership

Position Summary

Premier Family Medical is seeking a licensed or associate Behavioral Health Manager to join our primary care team under the Collaborative Care Model (CoCM). This role provides brief, evidence-based behavioral health services to patients with mild to moderate conditions in an integrated medical setting.

The ideal candidate is comfortable in a fast-paced primary care environment, values team-based care, and enjoys working collaboratively with medical providers.

Key Responsibilities

Provide brief behavioral health assessments, interventions, and follow-up visits for patients referred by primary care providers
Deliver evidence-based treatments such as CBT, behavioral activation, motivational interviewing, and problem-solving therapy
Collaborate closely with PCPs, care managers, and consulting psychiatric providers as part of the Collaborative Care Model
Participate in regular case reviews and treatment planning meetings
Monitor patient progress using standardized tools such as PHQ-9 and GAD-7
Document clinical encounters accurately and timely in the electronic medical record
Assist with care coordination, referrals, and patient education as needed
Maintain compliance with licensing requirements, clinic policies, and payer documentation standards
Support the integration of behavioral health services within the primary care setting

Qualifications

Current Utah license as ACMHC, LCMHC, LCSW, or LMFT (or eligibility for licensure)
Experience or strong interest in integrated behavioral health or primary care settings
Knowledge of evidence-based behavioral health interventions
Strong communication and collaboration skills
Ability to manage a caseload focused on brief, goal-oriented visits
Comfort working with diverse patient populations

Preferred Qualifications

Experience working within the Collaborative Care Model
Familiarity with primary care workflows and EMR systems
Experience using standardized behavioral health screening tools
Bilingual skills are a plus

Work Environment

This role is embedded within a primary care clinic and involves close collaboration with medical providers and staff. The position requires flexibility, strong organizational skills, and the ability to adapt to changing clinic and patient needs.
