DECLARE WELLNESS CENTER is an independent functional medicine practice in Murrells Inlet, SC, specializing in bioidentical hormone replacement therapy (BHRT), hormone optimization, and membership-based wellness care. 3. ABOUT THIS POSITION
The Health Coach will work closely with our team to guide patients through their wellness programs, lead health-related classes and workshops, and provide education on key lifestyle pillars including nutrition, movement, sleep hygiene, and stress management. You'll coach primarily in a group setting, with individual sessions when a patient needs extra support, helping them feel heard, motivated, and empowered to meet their goals.
This is a cross-trained role that combines health coaching with Wellness Coordinator responsibilities. Alongside supporting patients in their health journey, you will also assist with portal communication, patient follow-up, community management, and front desk coverage. We’re looking for a collaborative team player who is comfortable stepping in where needed to support the practice and patient experience.
Our protocols and processes are already established, so your primary focus will be helping patients successfully navigate and follow their care plan. You’ll thrive in this role if you enjoy working within a proven system, building strong patient relationships, and delivering consistent, high-quality support.
4. KEY RESPONSIBILITIES
Group Coaching & Patient Education
● Lead two one-hour group coaching sessions each week for active program members.
● Deliver the New Member Orientation class on Friday mornings for incoming patients.
● Run topic-based workshops covering detox, mindfulness, lifestyle, and biohacking.
● Educate patients across the core lifestyle pillars - nutrition, movement, sleep hygiene, and stress management.
● Schedule and conduct individual coaching sessions when a patient needs support beyond the group setting.
Behavior Change & Patient Follow-Through
● Identify the barriers keeping a patient from sustaining their plan, and work with them on realistic ways through.
● Respond to patient portal messages and keep communication moving between scheduled touchpoints.
● Conduct post-PME follow-up calls to check in on progress and address early obstacles.
● Review genetic reports and, where applicable, InBody scan results with patients, translating findings into practical next steps.
● Track patient engagement across the program and flag anyone who may need additional support.
Clinical Collaboration & Scope
● Work alongside the Provider to align lifestyle coaching with each patient’s medical treatment plan.
● Operate within your defined scope of practice, referring clinical questions to the Provider or Medical Director.
● Follow Declare’s established protocols and guide patients along a proven path of transformation.
● Participate in daily morning huddles, team training (typically held on Fridays), and case reviews with practice leadership.
● Complete Declare’s three-month patient program as part of your onboarding.
Patient Experience & Practice Support
● Manage our private Facebook group for members, encouraging interaction and community.
● Follow up with seminar and marketing leads to move interested prospects toward enrollment.
● Collect patient testimonials and coordinate thank-you gifts, cards, and birthday recognition.
● Contribute to other clinic initiatives as the practice grows.
Wellness Coordinator Cross-Training & Administrative Support
- Cross-train in the Wellness Coordinator role and assist with core administrative responsibilities alongside health coaching duties.
- Provide front desk coverage during staff absences, enrollment appointments, and busy periods.
- Support day-to-day practice operations as needed, demonstrating flexibility and a collaborative, team-first approach.
5. SKILLS & QUALIFICATIONS
Required
- Current health coaching certification (we do not require a specific certifying body)
- Minimum 6-12 months of hands-on health coaching experience
- Working knowledge of the lifestyle factors that drive health outcomes - nutrition, movement, sleep, and stress
- Confidence leading group sessions and speaking in front of a room
- Clear understanding of a health coach’s scope of practice and comfort working under the direction of the Clinic Director and Medical Director
- Strong written communication for portal messages and patient follow-up
- Proficient with Zoom, Calendly, Google Workspace, and Microsoft Office
- Genuine, personal passion for health and wellness - you actively seek out learning on your own time
- Personal engagement in health optimization - you live the habits you coach
- Willingness to cross-train in the Wellness Coordinator role and take on administrative duties
- Ability to connect with patients across a wide range of ages, goals, and starting points
Preferred (Nice to Have)
- Exposure to functional medicine, hormone health, or root-cause protocols
- Familiarity with behavioral change principles and motivational techniques
- Experience interpreting genetic reports, InBody results, or similar assessment tools
- EHR experience
- Background in personal training or fitness instruction
- Background in a cash-pay or membership-based practice model

7. COMPENSATION & BENEFITS
Pay: $27-$33 per hour, based on experience
Bonus Structure: Performance-based bonus; eligible after the 90-day introductory period
Schedule: Full-Time, five days per week.
- Monday–Thursday: 8:15 AM–4:30 PM
- Friday: Abbreviated schedule, currently 10:30 AM–12:30 PM, primarily for New Member Orientation.
- Every other Tuesday: Extended hours from 8:15 AM–7:30 PM to support the evening group coaching class.
Friday Schedule: Friday is a dedicated workday with shorter hours. The orientation start time may vary based on practice needs, and additional Friday hours may be required for team training and onboarding, particularly during the first few months. Friday hours may evolve as training is completed and the needs of the role develop

A note on benefits: Declare Wellness Center is a small, independent practice, which means we do not currently offer employer-sponsored health insurance.

Our team members typically obtain coverage through a spouse or partner, the ACA marketplace, or a health-sharing plan, and we are happy to discuss these options during the interview process.

By the six-month mark, success looks like consistent attendance across group coaching, a workshop calendar that runs consistently, patients who reach out to you between visits because they know you will answer, and an active members’ community that keeps people engaged with the program.
