Full Job Description

Description:

Position Summary: The role of the case manager is to undertake assessment, monitoring, symptom management support, goal planning, advocacy and linking of the consumer with rehabilitation and support services.

Clinical supervision provided to associates gaining hours toward licensure.

Duties and Responsibilities:

This person will work full-time in a direct service capacity. What follows is a list of duties and expectations for the position.

Read the Mentis Employee Manual. Implement program goals and objectives.
Meet regularly with the Program Manager and other case managers to clarify client needs, to problem solve, and to coordinate services.
Submit vacation requests, time sheets and mileage sheets to the Program Manager.
Attend and participate in monthly agency staff meetings.
Participate in weekly treatment team meetings to go over progress, challenges, and offer support with how to best work with clients.
Provide liaison support between clients and the landlords in our housing programs.
Collaborate with other providers to manage case coordination of assigned clients.
Co-facilitate weekly house meetings with other case managers.
Co-facilitate and plan wellness activities for clients across housing programs.
Schedule individual client meetings at a minimum of once a month. Meet clients in their unit or at Mentis campus a minimum of once a week. Caseload of approximately 15 clients. The caseload varies and can be up to 20 clients at a time.
Provide ongoing guidance, support, advocacy, and resource linkage to address symptom management needs, client acquisition of skills, meet stated goals, maintain stability and placement in housing program.
Track client appointments, goal updates, and ROI updates for clients.
Provide support to clients in resolving personal issues or problems as needed. Respond to clients when in an emergency or crisis.
Meet with clients every six months to review client progress towards achieving identified goals and to review client’s participation in the program.
Alert Program Manager as soon as possible when a client has an emergency or is in crisis.
Work with Program Manager to collaborate on client behavioral issues as needed.
Support clients in working through and resolving conflicts with neighbors or housemates.
Work collaboratively with others to facilitate consistency and coherency among client’s various service providers, (i.e. doctors/psychiatrists, social workers, Social Security Administration, County Eligibility Department, Vocational Rehabilitation, and some family members, among others.) Arrange case conferences when needed; this should be thought through carefully with Program Manager.
Support client’s in applying for housing opportunities.
Assist client’s in move in/move out.
Write a weekly progress note for each client after each scheduled individual meeting. Document client contact and collaborative contacts.
Write weekly house meeting note for assigned house (rotates quarterly). Write note for each client, even if client is not present at house meeting.
Maintain clinical documentation according to the standards of the profession and as instructed by program manager.
Document identified cases according to MediCal standards when applicable.
Participate in annual MediCal compliance training.
Attend provider meetings in the community that are applicable to the client’s treatment, care and wellbeing as requested by the Supportive Living Programs Manager.
Provide and model appropriate boundaries and clear communication.

Minimum Qualifications:

Graduate student in a related field, or completion of Master’s Degree in Social Work, Psychology or Counseling Preferred.
Registration as an ASW. AMFT, or APCC
Experience working with mentally ill adults, preferably in a residential setting.
Experience in a Medi-Cal funded program is a plus.

Compensation: $30-34hr.

California Pay Transparency Disclaimer: This is an Exempt position. The pay range for this position is $30/hour - $34/hour.

Actual placement within range will be contingent upon several factors, including but not limited to the candidate's qualifications, education, experience, internal equity, and alignment with market data.
