We are offering sign on bonuses of $10,000 for full-time RN's

Job Responsibilities:

Provide admission, evaluation and follow-up skilled nursing visits for home health patients
Provide quality hands-on care, management and teaching of patient’s care plan in accordance with physician orders, under Clinical Manager's supervision
Coordinate appropriate care, involving all the other healthcare team members (such as Physical
Therapists, Occupational Therapists, Home Health Aides and Medical Social Workers)
Report patient’s progress to physician and Clinical Manager as required to maintain continuity of care
Implement plan of care with patient and family to assist them in achieving optimal outcomes

Qualifications:

Valid RN License to practice in CA
Current BLS/CPR certification
Valid CA Driver’s license
Minimum of one year nursing experience is preferred
Med Surg, ICU, ER, Acute Experience, Home Health experience is a plus

***Wound Care experience preferred. ***
