Full Job Description

Clinical Care Coordinator – School-Based

Reports To: Manger, School Based Programs

Position Summary

Clinical Care Coordinators will coordinate the referral and linkage to services and resources both within and outside of FCS. In the school-based program context, Clinical Care Coordinators also serve as the primary point of coordination and administrative oversight for intake of school-based clients, including managing referrals, tracking service hours and case assignments, and ensuring smooth transitions across providers. Clinical Care Coordinators will work collaboratively with all other departments in the organization and will provide excellent customer service. The Clinical Care Coordinator will also be responsible for the effective supportive counseling, case management and community support of agency clients. Schedule flexibility is expected, and some evening/weekend hours may be required.

Responsibilities

Coordinate referrals and connect students and families with internal and community-based services.
Manage the intake process for school-based behavioral health services.
Schedule initial appointments and coordinate transitions between providers.
Track referrals, assessments, case assignments, and service hours.
Collaborate with schools, families, clinicians, and community partners to ensure continuity of care.
Assist clients with accessing community resources such as housing, food assistance, insurance, healthcare, employment, and educational supports.
Provide supportive counseling, case management, crisis intervention, and treatment planning as needed.
Complete accurate and timely clinical documentation.
Assist with service authorizations required by insurance or payer sources.
Cross-train team members on school-based intake processes to ensure seamless coverage.
Participate in staff meetings, agency initiatives, and community outreach activities.

Qualifications

Required

Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field with at least one year of clinical experience, or
Master's degree in a related field.
Minimum of one year of clinical experience.
Strong organizational, communication, and customer service skills.
Ability to manage multiple priorities in a fast-paced environment.
Proficiency with Microsoft Office applications.

Preferred

Bilingual (English/Spanish or other languages).
Experience working in schools, behavioral health, or community mental health settings.

Work Environment

Local travel only (less than 10%).
Flexible schedule with occasional evening or weekend hours as needed.
Ability to sit, stand, walk, and use a computer throughout the workday.
Ability to lift up to 25 pounds occasionally.

Wage

$42,000 - 50,000 (based on experience)
