Chronic Care Management (CCM) Case Coordinator

Employment Type: Full-time / Part-time

About the Role

MEDOCSCare Management Services is looking for a dedicated CCM Case Coordinator to support our Chronic Care Management program. In this role you will serve as the primary point of contact for enrolled patients, coordinating their care between visits, monitoring chronic conditions, and ensuring patients receive the minimum required care management minutes each month in compliance with CMS guidelines. You will work closely with our RN care managers and physician partners to keep patients engaged, informed, and on track with their care plans.

What You Will Do

Conduct monthly outreach calls to enrolled patients to review their chronic conditions, medications, and care plan progress
Document all patient interactions accurately and in real time within the EHR, meeting the required CCM time thresholds each month
Coordinate with primary care providers, specialists, and pharmacies on behalf of patients to close care gaps
Schedule and confirm upcoming appointments, referrals, and lab work
Identify patients who are non-responsive or at risk of falling out of the program and escalate to the RN or physician as needed
Educate patients on their chronic conditions, medications, and self-management strategies
Maintain accurate patient rosters and monthly billing documentation to support CCM claims
Flag any urgent clinical concerns to the supervising RN or provider immediately
Support enrollment calls for new patients being onboarded into the CCM program

Required Qualifications

Certified Medical Assistant (MA) or Certified Nursing Assistant (CNA) credential required
Minimum 1-2 years of experience in a medical office, clinic, or care coordination setting
Working knowledge of common chronic conditions such as diabetes, hypertension, COPD, and CHF
High school diploma or equivalent required; associate degree in health administration or related field a plus
Comfortable working within an EHR system and learning new software tools quickly
Strong phone communication skills and comfort speaking with elderly or medically complex patients
Excellent documentation habits and attention to detail, since CCM billing depends on accurate time tracking

Nice to Have

LPN or RN license
Experience working specifically with CMS Chronic Care Management billing codes (99490, 99439, 99487, etc.)
Bilingual candidates (Spanish/English)
Background in a remote or distributed healthcare team

Compensation & Schedule

Compensation will be discussed based on experience level. This is a remote position with flexible scheduling, though availability during standard US business hours is preferred to align with patient outreach windows.
