Position Summary
The MDS Coordinator plays a vital role in ensuring the accuracy and integrity of the Resident Assessment Instrument (RAI) process while maximizing reimbursement and maintaining regulatory compliance. This position is responsible for coordinating and completing all MDS assessments, facilitating care planning, supporting PDPM reimbursement, and working closely with the interdisciplinary team to ensure residents receive the highest quality of care.
Key Responsibilities
- Coordinate, complete, and submit accurate and timely MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations
- Lead and facilitate the interdisciplinary care planning process
- Ensure compliance with all federal, state, and company documentation requirements
- Review clinical documentation for accuracy and reimbursement optimization under PDPM
- Collaborate closely with nursing leadership, therapy, physicians, and other departments to ensure comprehensive resident assessments
- Monitor assessment schedules and maintain timely completion of all required documentation
- Identify opportunities for clinical documentation improvement and reimbursement enhancement
- Assist with regulatory surveys, audits, and quality assurance initiatives
- Maintain expert knowledge of CMS regulations, MDS 3.0, RAI guidelines, and reimbursement changes
- Serve as a clinical resource for nursing staff regarding documentation and assessment processes
Qualifications
- Current RN or LPN license in good standing
- Required: Recent MDS Coordinator experience in a Skilled Nursing Facility or CCRC
- Strong knowledge of MDS 3.0, RAI process, PDPM, Medicare, and Medicaid regulations
- Experience with electronic medical record (EMR/EHR) systems
- Excellent assessment, documentation, organizational, and communication skills
- Ability to work collaboratively with an interdisciplinary team
- RAC-CT certification is a plus, but not required
