Full Job Description

Position Overview

Life Coping Inc. has a Part Time opportunity for an experienced Case Manager with an RN, SW, MSW Social Services degree or relevant experience to work with chronically ill elderly and adult disabled adults in the Manchester/Nashua areas. Knowledge and experience with "Choices for Independence" Medicaid Home and Community Based waiver required.

Scope of Services

Provide independent case management to New Hampshire seniors and adults with chronic illnesses and/or disabilities, substance use disorder, and behavioral health diagnosis
Performing comprehensive needs assessment, coordination of services, service monitoring, transitions, advocacy.
Assist clients and families in making informed decisions about program options and service providers provided by Medicaid, Medicare and insurance
Establishing goals and desired outcomes
Preventing unnecessary trips to hospital emergency department

Training

A full training program is provided by our clinical director and case management staff as necessary.

Founded in 1994, Life Coping (LCI) is the largest independent case management agency in New Hampshire. LCI provides independent integrated case management to New Hampshire seniors and adults with chronic illnesses and/or disabilities according to applicable state regulations. LCI provides and integrated and whole person approach to case management covering a persons physical, Work is performed with a high degree of independence and autonomy.

Our mission is to assist individuals to remain safely and independently in their home and the community. Our Staff of nurses and social workers works with the elderly, adult disabled, developmentally disabled. We work closely with the New Hampshire Department of Health and Human Services as a licensed case management agency in assessing and accessing home care services and community supports thousands of individuals on Medicaid throughout the State of New Hampshire. Among our specialties is transitioning a person from an institutional setting such as a nursing home, hospital, rehabilitation center or a psychiatric hospital. Life Coping is recognized for leadership and excellence in providing case management services.
