Superior Caregivers is a leading provider of services and supports to people with intellectual and developmental disabilities. We provide services to youth who are in the custody of the TN Department of Children’s Services. Each youth is assigned a Superior Caregivers Program Coordinator who oversees the youth’s services through Superior Caregivers. The main difference between family model and supported living is the setting.
This position will be a Program Coordinator for youth in Foster Care located in Nashville, TN. Position pays $58,656 per year via an exempt salary.

Minimum Requirements:
- Bachelor’s Degree in Social Work, Psychology or equivalent in closely related field and experience in the child welfare system; Master’s degree preferred.
- Must maintain professional licensure as required by degree, when applicable.
- Must possess professional and expert knowledge of communications, as well as building and maintaining relationships.
- Must be able to travel frequently within assigned region, including the organizations designated office.
- Strong organizational abilities including planning, delegating, program development and task facilitation.
- Must be able and willing to work some hours outside of normal working hours to respond to emergencies or to otherwise meet the needs of the people we serve and their families.
- Must have consistent access to reliable transportation.
- Must possess a valid state Driver’s License.
- Must pass a motor vehicle record (MVR), background check and other screening per Superior Caregivers requirements.
- Caseload total: 10 children/youth.
Job Qualifications:
This position is responsible for providing on-going case management to the child/youth placed in a Superior Caregivers Foster Care homes. The position requires knowledge in TN DCS policy, ability to collaborate within a team, and work in conjunction with Superior Caregivers’ mission and values. This includes meeting with participants, families, guardian, and other key stakeholders to explore opportunities and deliver quality services. This position exists to provide on-going case management that ensures all child/youth’s needs are met timely and efficiently with quality services. Maintain a professional appearance and positive attitude at all times while working with persons in services and representing Superior Caregivers.
- Solid spoken and written communication skills.

Essential Functions and Responsibilities (included, but not limited to):

- Enter quality documentation timely and efficiently into TFACTS.
- Learn and maintain a basic understanding of TFACTS.
- Attend Child Family Team Meetings (CFTMs) and actively participate.
- Ensure assigned child/youth receives services, as appropriate/required.
- Provide all documentation to the appropriate DCS staff and store in child/youth’s internal case file.
- Prepare monthly summaries and enter them in DCS within policy deadlines.
- Completes URs monthly and participates in internal URs monthly.
- Attends team and agency meetings, as appropriate.
- Provides support to foster families after hours.
- Occasionally transports assigned child/youth to appointments, change in placement, visitation, court, etc.
- May be required to transport youth as needed to and from Foster Care homes.
- Maintain child case files that are in compliance with DCS and Superior Caregiver’s policy requirements.
- Completes annual training.
- Maintain general knowledge of foster parent policy and rights to ensure their rights are upheld.
- Completes foster home compliance checks routinely to ensure the home is safe and in compliance with DCS policy 16.8.
- Be available to foster families and communicate any needs timely and efficiently.
- Work in partnership with all community partners; attending any meeting(s), as appropriate.
- Perform other such duties as assigned by the Executive Director of Superior Caregivers, or designee, that are within the scope of the position, as well as those reflective of professional experience, education, and ability.

Physical Demands and Working Conditions:
1. Physical Requirements - Requires repetitive manual and finger dexterity and eye- hand coordination; requires the ability to use all office equipment; and the ability to lift/carry up to 30 pounds using appropriate body mechanics.
2. Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Must be able to communicate verbally with all staff, vendors, people we serve and clients.
3. Pressure Factor – May require working under stressful conditions at times and handling emergencies. Working conditions may be noisy with fluctuating indoor temperatures. There will be pressure at times to meet scheduled appointments and deadlines and manage crisis situations. This position will interact with people we serve who are intellectually and developmentally delayed and maybe frail or confused and have the potential for verbal or physical aggression.
