Full Job Description

Position Summary

The Family Nurse Practitioner (FNP) provides medical services to women participating in Mending Hearts’ full continuum of care, including detox, residential, and outpatient programs. This role requires advanced clinical competence, strong decision-making, and collaborative teamwork with medical, behavioral health, and peer support staff.

In alignment with grant-funded priorities, the FNP oversees and administers Medication-Assisted Treatment (MAT) services, including Vivitrol/Naltrexone injections, as part of Mending Hearts’ outpatient and after-care programs. The FNP plays a vital role in supporting continuity of care and ensuring women transition successfully from inpatient treatment into long-term recovery.

Essential Duties & Responsibilities

Perform comprehensive medical evaluations at intake and throughout the continuum of care.
Develop and adjust individualized treatment plans in collaboration with the Medical Director and behavioral health team.
Administer and monitor MAT services (e.g., Vivitrol/Naltrexone), ensuring compliance with documentation and grant reporting requirements.
Provide medication management, monitor side effects, and educate clients and families about psychiatric and medical treatments.
Support continuity of care by coordinating with peer support staff, alumni programs, and case managers.
Supervise nursing and medical assistant staff, ensuring adherence to clinical protocols and quality standards.
Oversee detox unit operations, including staffing, safety, and compliance.
Provide 24-hour medical on-call coverage as scheduled.
Lead infection control, disease prevention, and health promotion efforts (e.g., TB testing, COVID protocols).
Participate in staff and clinical meetings as a member of the leadership team.
Refine and update treatment protocols under the guidance of the Medical Director.
Ensure ethical practice and compliance with state/federal regulations and grant requirements.

Qualifications

Master’s Degree in Nursing and ANCC certification as a Family Nurse Practitioner (FNP).
Current Tennessee RN license and TN APN certificate with certificate of fitness to prescribe.
DEA registration required.
Minimum of 2 years’ clinical experience, preferably in addiction medicine, behavioral health, or family practice.
Experience supervising nursing staff and using electronic medical records.
Demonstrated ability to provide care across the lifespan and collaborate with multidisciplinary teams.
