Full Job Description

OVERVIEW

The Director of Pharmacy serves as Pharmacist-in-Charge (PIC), holding clinical, compounding, and regulatory ownership of the pharmacy. Accountable for the clinical soundness of every therapy, the integrity of sterile compounding under USP 797, and the pharmacy's standing with ACHC and every state board of pharmacy where licensed.

RESPONSIBILITIES

Clinical Leadership

Clinical oversight of home parenteral nutrition (TPN, IDPN, IPN) - formulation accuracy, compatibility, stability, patient-specific dosing
Final clinical verification of compounded sterile preparations
Senior clinical resource for pharmacy staff and referring physicians
Maintain clinical protocols and beyond-use dating standards

Sterile Compounding Compliance & Quality

Own end-to-end USP 797 compliance: facility/engineering controls, personnel training, garbing/aseptic technique, environmental monitoring, media fill testing, competency assessment
Maintain SOPs, master formulation/compounding records, and keep documentation survey-ready
Oversee sterility, endotoxin, and stability testing to support beyond-use dating
Maintain quality management system: deviations, root cause analysis, CAPA, adverse event reporting
Lead patient safety and medication error management
Own clinical/compounding policy set (author, review, approve, version-control)
Maintain USP 800 hazardous drug readiness
Ensure compliance with FDA Section 503A compounding requirements

Regulatory, Accreditation & PIC Accountability

PIC of record; maintain good standing with New York State Board of Pharmacy
Own ACHC accreditation (and others): survey readiness, mock surveys, remediation
Own compliance across 30+ state boards of pharmacy (resident/nonresident), partnering with licensing function on applications/renewals
Ensure DEA compliance for controlled substances
Maintain drug integrity: supplier qualification, DSCSA traceability, recalls/quarantines
Ensure compounding/dispensing/billing compliance with CMS/Medicare, including DMEPOS supplier standards

Clinical Team Leadership

Supervise, mentor, and develop pharmacists and technicians
Build clinical staffing depth beyond single-person dependency
Participate in/oversee pharmacist on-call rotation (alternating weekends, holidays)

Financial & Budget Management

Develop and Manage operating/capital budgets (drug, supply, labor, equipment)
Oversee inventory: par levels, expiry/waste control, accuracy
Monitor cost of care by therapy line; identify efficiencies
Evaluate and justify capital equipment acquisitions

Systems & Compounding Technology

Maintain data integrity in pharmacy management/dispensing platforms (e.g., Brightree)
Oversee automated compounding device calibration and verification, and integration with compounding records
Manage infusion device fleet (ambulatory pumps, enteral feeding pumps)

QUALIFICATIONS

Required

PharmD or equivalent
Active, unrestricted NY pharmacist license, PIC-eligible
5+ years sterile compounding/infusion pharmacy experience; working knowledge of USP 797
3+ years supervising/developing pharmacy staff
Strong clinical foundation in nutrition support, home infusion, or related specialty

Preferred

Home infusion/home parenteral nutrition experience
ACHC accreditation familiarity
Board certification (BCNSP or BCPS)
Multi-state licensure or willingness to pursue
CMS/Medicare reimbursement & coverage for compounded and infused therapies familiarity
Budget management experience

WORK ENVIRONMENT

On-site role in a sterile compounding facility, including cleanroom. Requires ongoing garbing/aseptic competency and shares in the pharmacist on-call rotation (evenings, alternating weekends, holidays).

Salary matched to qualifications and experience.

Pay: $155,000.00 - $175,000.00 per year
