Square Medical Group’s (“Square” or “SMG”) mission is to deliver enduring, material improvements in both clinical and wellness outcomes to patients addressing co-occurring challenges with substance use disorder (SUD) and behavioral health (BH). We do this through delivering a specialized continuum of outpatient care of 1,000+ units/week to a population of 5,000 Members across Massachusetts: medication management, in-person and virtual therapy for individuals, couples, and families, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as three levels of housing.

Program Overview :

The residential recovery programs are structured rehabilitation-oriented treatment focused on trauma-informed care and harm reduction. It is designed for motivated individuals who want to enhance the quality of their day-to-day lives. Through active engagement in groups and the development of the community, we will work together to overcome challenges that interfere with our abilities to live life to the fullest.
Members will be assigned a weekly life skill to ensure ongoing cleanliness/maintenance of the house. Members are expected to take part in assigned life skills. Staff will help if Members are unsure about how to complete a task. Life Skills are necessary to independent living.
On average, Members spend between three to six months in the three phases of the residential program. Each phase of the program rewards Members with increasing levels of responsibilities and freedoms. Throughout the program, Members are supported by deep expertise from SMG’s outpatient services, our core capability. Members are also able to stay with our outpatient practice post-discharge for continuity of care.
Role Summary:
As a Case Manager, you will play a pivotal role in supporting Members with complex behavioral health and substance use needs as they work toward stability and reintegration. This is not a paperwork-only role, you will build trust, foster therapeutic engagement, and serve as a fierce advocate for Members navigating housing, healthcare, legal, vocational, and social systems. Using a harm reduction and trauma-informed lens, you will coordinate individualized services, remove real-world barriers, and empower Members to take ownership of their recovery journey. Your work will directly shape each Members’s path toward independence, one meaningful connection at a time.

You will play a vital role in delivering high-quality, person-centered behavioral health care to a diverse adult population. Your responsibilities will require flexibility, creativity, and collaboration to ensure each Member receives care that meets their unique needs and goals.
1. Develop individualized service plans in collaboration with clinical and housing teams
a. Build strong, trust-based relationships with Members and act as a consistent point of support throughout their program stay
b. Conduct holistic assessments of Members needs across behavioral health, housing, medical, financial, educational, and vocational domains
c. Deliver one-on-one coaching that promotes life skills, recovery goals, and self-efficacy
2. Coordinate referrals and connections to community services, providers, and benefits
a. Support discharge planning and post-program transitions to outpatient care or stable housing
b. Facilitate sober support building and engagement in community recovery resources
3. Contribute to a positive, supportive team culture that values communication, compassion, shared learning, and a commitment to SMG’s mission
a. Participate in team huddles, case reviews, and interdisciplinary planning sessions
b. Maintain timely documentation in the EMR, track outcomes, and support quality improvement efforts
c. Model professional communication, mutual respect, and recovery-oriented behavior in all Member interactions
Specific Tasks:
The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Care coordination
a. Conduct and document initial case management assessments focused on behavioral health, housing, income, healthcare access, education, and social supports
b. Develop individualized, strengths-based service plans in collaboration with the clinical team and Members
c. Provide one-on-one coaching to support life skills development, self-advocacy, and goal achievement
d. Engage in routine check-ins with Members to monitor progress and adjust service plans as needed
2. Assist Members in applying for housing, benefits, transportation, job training, and primary care services
a. Support discharge planning by identifying step-down care options and preparing necessary documentation
b. Support Member insurance authorizations and care coordination
c. Coordinate and track referrals to external service providers and community-based resources
3. Link the Member with any other staff or services (i.e. Linkage), including those outside the house and outside the SMG, to achieve the goals of the plan
4. Monitor the progress of the Member and adjust the plan as needed
5. Advocate for the Member to achieve and utilize necessary services
6. Team coordination
a. Participate in weekly staff meetings, clinical case reviews, and Members phase progression discussions
b. Maintain timely and accurate documentation in the electronic medical record (EMR), including progress notes, assessments, and service logs

Qualifications:
Required:
o Working knowledge of behavioral health recovery principles, harm reduction, trauma-informed care, and social determinants of health
o Familiarity with housing, employment, healthcare, and social services across Greater Boston
o Strong communication, organization, and problem-solving skills
o Proficiency with basic technology and EMR systems
Preferred:
o Associate’s degree in Psychology or related field + 1 year of relevant experience, OR 3 years of related experience in lieu of degree
o Experience working in residential treatment, behavioral health, or substance use settings
o Bilingual or multilingual fluency (Spanish, Portuguese, Haitian Creole)

ATTENDANCE: Regular job attendance is an essential job function
Schedule:
- Full-time, 40 hours per week
- Days and hours are flexible as long as performance metrics set by Manager are achieved
