Job Summary
The Diversion Specialist Case Manager assists individuals and families experiencing homelessness or at risk of homelessness by providing comprehensive case management and support services. This includes conducting assessments, developing individualized service plans, connecting clients to community resources, and supporting them in achieving stability and independence.
Duties/Responsibilities
- Conduct intakes and assessments to evaluate client needs and develop individualized service plans.
- Provide case management services, including referrals and advocacy for essential resources and community services.
- Facilitate linkages to housing, medical care, behavioral health services, and other supports based on client goals and needs.
- Assist clients with developing skills to manage day-to-day challenges, including budgeting, transportation, and accessing public benefits.
- Provide transportation support to clients as needed to facilitate access to services.
- Conduct regular follow-ups to monitor client progress and reassess service plans as necessary.
Requirements
- Associates Degree in Human Services required; Bachelors in Human Service filed preferred.
- Bilingual (Spanish/English) preferred
