Full Job Description

Required Education and Experience

Bachelor’s Degree in Social Work, Psychology, Counseling, or a related human services field.
Minimum of one (1) year of professional experience providing direct services to children and families, preferably in a care management, case management, or behavioral health setting.
Master's degree in social work or related human services field preferred.

Additional Qualifications/Attributes

Exceptional organizational abilities, time management, attention to detail, and the ability to prioritize multiple tasks.
Strong interpersonal and communication skills, with the ability to engage families and collaborate with professional partners.
Valid driver’s license in good standing and access to a reliable vehicle.
Demonstrated capacity for cultural competence and sensitivity to diverse family needs.
Bilingual (preferred) – ability to communicate effectively with families in their preferred language is highly valued. (Spanish, Creole, Portuguese, ASL, etc.)
Ability to work both independently and as part of a team.
Knowledge in Microsoft Office 365 (Word, Outlook, PowerPoint, Excel)
Comfortable using electronic health record systems and documentation software (e.g., CYBER).
Availability for occasional evening or weekend work, including participation in on-call rotation for crisis management coverage.

Job Summary

The Care Manager plays a central role in building meaningful partnerships with children and families while coordinating care with system partners. This position serves as a key facilitator of the Child and Family Team (CFT) process, empowering families through strength-based, individualized planning. The Care Manager works collaboratively to ensure that all supports and interventions align with the youth’s goals, ensuring continuity of care and access to the least restrictive, most effective services.

The ideal candidate is passionate about working with youth and families, understands the complexities of behavioral health needs, and is committed to the values of equity, empowerment, and positive outcomes. This is a community-based position requiring regular travel to family homes, service provider locations, and community-based settings. This position may involve exposure to emotionally challenging situations and requires emotional resilience, cultural sensitivity, and a strong sense of professional boundaries.

Essential Functions and Responsibilities

Review referral packets and all relevant assessments prior to the initial engagement with the child and family.
Participate in initial family meetings to:
Build rapport and trust with the youth and caregivers.
Introduce the wraparound process and the development of an Individualized Service Plan (ISP).
Collaboratively develop a comprehensive crisis plan.
Facilitate the coordination and scheduling of ongoing Child and Family Team (CFT) meetings.
Maintain a proactive approach to care by identifying team members, linking the family to resources, and revising ISPs and crisis plans as needed.
Ensure timely access to appropriate formal and informal services to address identified needs.
Provide ongoing support by meeting with youth and families at least once monthly in settings such as their homes, community locations, or office.
Monitor service delivery, ensure quality of care, and track progress toward established goals.
Document all contacts, progress notes, and interventions in CYBER or designated systems on a weekly basis.
Provide scheduled on-call crisis management coverage to support families during times of urgent need.
When out-of-home treatment is necessary:
Initiate and complete referral packets, upload documents to CYBER, and liaise with the Specialty Residential Treatment Unit.
Facilitate “meet and greets” between families and providers.
Maintain up-to-date records in YouthLink and coordinate Joint Care Reveiw Plans as required.
Complete the Strengths and Needs Assessment (SNA) and maintain current Care Manager certifications annually.
Ensure that ISP discharge or transfer plans are completed within two (2) business days following the final CFT meeting.
Submit flex funds requests and referrals in accordance with agency timelines.
Comply with all reporting requirements including the Universal Incident Reports (UIRs) and documentation of child abuse/neglect per mandated reporting laws.
Maintain frequent communication with provider agencies and family members to ensure effective collaboration and continuity of care.
Attend court hearings or legal proceedings involving the youth, offering support and relevant documentation when needed.
Identify and report gaps in community services to supervisory and resource development teams. Contribute to the agency’s knowledge base by identifying new resources and fostering strong ties with service providers.
Participate actively in team meetings, in-service training, agency-sponsored workshops, and conferences to stay current with best practices.
Meet regularly with your supervisor to review caseload progress, seek guidance, and troubleshoot service barriers.
Maintain a professional demeanor and represent the agency’s mission and values in all interactions.
