Full Job Description

Position Summary:

The Admissions Coordinator manages and oversees the admissions process for new and returning residents in the nursing home. This role ensures that all residents and families experience a smooth, welcoming, and organized transition into the facility. The Admissions Coordinator works closely with the Administrator, Director of Nursing, Business Office Manager, and Social Services to coordinate clinical, financial, and psychosocial aspects of each admission while maintaining regulatory compliance and positive community relations.

Essential Duties and Responsibilities:

Admissions Process Management:

Coordinate all aspects of the admissions and pre-admissions process, from initial inquiry to room assignment.
Respond promptly and professionally to all phone calls, walk-ins, hospital referrals, and community inquiries.
Conduct facility tours for prospective residents and families, highlighting programs, services, and amenities.
Review and process admission applications, ensuring all required documentation (clinical, financial, and legal) is complete prior to admission.
Collaborate with nursing and therapy departments to ensure residents are appropriately placed based on their clinical needs.
Coordinate transportation and communicate move-in details to all involved departments.
Maintain an accurate census and bed availability report.

Resident & Family Relations:

Serve as the primary contact for residents and families throughout the admission process.
Provide education regarding facility policies, services, rates, and admission procedures.
Support new residents and families during the transition period, ensuring a welcoming and comfortable experience.
Work closely with Social Services to assist with adjustment issues and ensure a positive onboarding experience.

Clinical & Financial Coordination:

Collaborate with the Business Office Manager to verify insurance coverage, payer sources, and financial eligibility.
Work with the MDS Coordinator, Director of Nursing, and therapy staff to assess the appropriateness of each referral.
Review pre-admission screening forms and hospital discharge summaries for completeness.
Ensure compliance with Medicare, Medicaid, Managed Care, and private pay admission requirements.
Maintain accurate records for all admissions, transfers, and discharges in accordance with facility and regulatory standards.

Marketing & Community Relations:

Develop and maintain strong relationships with hospitals, referral coordinators, social workers, and other healthcare partners.
Participate in community outreach activities to promote the facility’s programs and services.
Maintain an updated referral contact database and conduct regular follow-up with referral sources.
Assist in developing promotional materials, brochures, and community presentations.

Regulatory & Documentation Compliance:

Ensure all admissions paperwork meets state, federal, and facility standards.
Maintain confidentiality of all resident and business information in accordance with HIPAA regulations.
Participate in Quality Assurance and Performance Improvement (QAPI) meetings and initiatives related to admissions.
Prepare monthly admissions reports and track marketing and referral trends.

Qualifications:

High school diploma or GED required; associate’s or bachelor’s degree in healthcare administration, social services, business, or related field preferred.
Minimum 1–2 years of experience in admissions, social services, marketing, or case management within a nursing home, hospital, or healthcare setting.
Strong understanding of Medicare, Medicaid, Managed Care, and private insurance processes.
Excellent interpersonal, organizational, and communication skills.
Ability to multitask, prioritize, and maintain professionalism under pressure.
Proficiency in Microsoft Office and experience with electronic medical record (EMR) or census systems.
Compassionate, customer service–oriented, and able to work effectively with residents, families, and staff.

Physical Requirements:

Ability to sit, stand, and walk for extended periods.
Must be able to occasionally lift up to 25 pounds.
Visual and auditory ability to interact with residents, families, and staff.

Work Schedule:

Full-time, typically Monday through Friday.
Must be available to assist with after-hours or weekend admissions as needed.
