Full Job Description

Position Summary: The Clinical Supervisor is responsible for supervising the provision of services that prevent or resolve crisis in order to prevent unnecessary use of emergency rooms and/or in-patient services, and assuring crisis intervention services 24 hours a day, 7 days per week.

Below are the primary responsibilities of a Team Leader:

Major Duties:

· Lead a team consisting of Senior Intensive Care Coordinators, Children’s Care Coordinators and Supportive Care Coordinators, promote a culture of collaboration and open communication among staff.

· Work with Care Coordinators and providers to assess clients’ needs and goals, and ensure that person-centered care plans are developed that include client’s strategies and objectives to address their identified needs.

· Supervise all aspects of the provision of services to clients including the identification of individuals appropriate for Children’s Care Coordinator.

· Ensure that coordination and contact with all service providers and support systems is maintained including but not limited to psychiatrists, therapists, state and local hospitals, community residences, court systems, family members, primary care providers, and educational/vocational service providers.

· Monitor client participation and satisfaction with services, and advice and advocate for clients and staff to assist with overcoming difficulties related to service delivery.

· Supervise the search for missing clients in the community following the lost to contact protocol.

· Ensure staff meets deadlines on submission of Continued Stay Reviews. Train staff as needed on Continued Stay Reviews.

· Ensure all required documentation relating to service delivery for each client meets NYSDOH, NYSOMH, OMIG, Health Home, and agency guidelines.

· Ensure direct line staff are meeting minimum quality requirements with regards to levels of service, signed consent forms, lost to contact clients, and documentation.

· Participate actively in supervision, performance management and professional development processes and activities for self, and promote and monitor participation for staff. Keep Director informed of program activities, trends, concerns and challenges, and seeks guidance and consults with Director regarding client or administrative issues. Provide quality supervision of staff including but not limited to case coverage, chart audits, clinical support and professional development.

· Monitor Case Coordinators’ compliance with administrative tasks: statistical reports, levels of service, weekly activity reports, progress notes, and agency policies and procedures.

· Adhere to agency policies and procedures as documented in the Employee Handbook.

· Participation in agency events and functions when required.

· Other duties as determined by the Program Director.

Qualifications:

· Master’s degree required.

· Five or more years’ experience in a leadership role managing corporate compliance in a non-profit setting.

· Strong expertise in Microsoft Office Suite.

· Must have superior organizational and analytical skills, and excellent verbal and written communication skills.

· Must be able to work independently and manage multiple tasks in a fast-paced environment.

· Ability to work collaboratively with a variety of people, as well as an aptitude for teamwork.

· Ability to use public transportation for travel to program locations throughout New York City.

Pay: $60,000.00 - $67,000.00 per year
