Who is Bridging Access to Care, Inc.: Bridging Access to Care, Inc. (BAC) is a nonprofit, multi-service, community-based behavioral health care organization committed to building healthy communities, one person at a time.

Brooklyn Aids Task Force was the very first HIV/AIDS organization in Brooklyn prioritizing serving persons living with HIV/AIDS (PLWHA) since 1986. In 2012, Brooklyn Aids Task Force changed its name to BAC and successfully made the transition from an AIDS-centered organization to a multi-service community-based healthcare organization. Today, BAC provides mental health and substance use-related social services, comprehensive HIV/AIDS care, and coordinated health care and supportive services to underserved racial and ethnic minority communities in Brooklyn and other parts of New York City.

JOB SUMMARY: The Health Educator, OASAS, delivers evidence-based substance use prevention programs and health education to youth ages 14–24, with a focus on alcohol and substance misuse, HIV/AIDS awareness, and healthy decision-making. Experienced in facilitating prevention curricula, coordinating youth advisory initiatives, creating health promotion campaigns, and connecting young people with supportive services. Adept at building collaborative partnerships with schools and community organizations while maintaining accurate records and ensuring program compliance, and thrives in fast-paced, field-based environments requiring extensive travel and flexible scheduling.

Success in this role requires strong group facilitation, relationship-building, communication, and organizational skills, and collaborating effectively with diverse populations and community partners. This role facilitates group interventions using outreach to schools, builds partnerships to engage youth, and provides culturally responsive education and referrals to appropriate health and support services. This position requires frequent field-based work, with up to 75% travel throughout the boroughs.

EDUCATION AND WORK EXPERIENCE:

MINIMUM: High School Diploma/ GED with at least three (2) years of professional experience in the field or with alcohol and substance misuse and HIV/AIDS issues, including engagement and motivation, and referral for testing and health care services.
Bachelor’s degree in health education or related field and one (1) year of relevant experience
MINIMUM: Two (2) years’ experience working in substance use group facilitation with the target population (youth 14-24)
Experience working with adolescents around alcohol and substance misuse education.
Experience with conducting outreach and networking which includes prioritizing locations and building and maintaining positive relationships with clients and companies.
Up to 75% of travel is required.

Licensure: A NYS Driver’s License with at least a two (2) year clean driving record, preferred

Certification: CASAC, preferred

DUTIES AND RESPONSIBILITIES:

Program Delivery & Interventions

Deliver evidence-based practice (EBP) interventions to fidelity, conducting all phases as outlined in the following curriculum:

1. Too Good for Drugs

2. Too Good for Violence

3. Life Skills

Create and manage a Public Service Announcement (PSA) per cycle to promote educational and informational messages on social media platforms.
Establish and facilitate a virtual Youth Advisory Board (YAB)

Partnerships & Outreach

Develop partnerships, Memoranda of Understanding (MOUs), and linkages to recruit the target population and deliver programs (e.g., schools, youth agencies).
Provide culturally appropriate and competent substance use prevention education and outreach to the youth population.

Administrative & Reporting

Maintain and update Outlook and eCR calendars regularly.
Maintain updated and accurate individual records for all clients via eCR and WITNYS
Maintain updated and accurate individual records for all clients via eCR.
Complete monthly reports and close client charts according to BAC and department standards.

Other duties assigned are based on program needs.

KNOWLEDGE, SKILLS, AND ABILITIES:

Previous experience in substance use group facilitation with the target population (youth 14-24)
Previous experience utilizing assessment tools such as the CRAFFT screener
Ability to work as a team member with people of various racial/ethnic orientations and religious backgrounds.
Ability to work with multiple targeted populations such as youth, HIV+, high-risk/at-risk individuals
Computer proficiency in Microsoft Word, Excel, and PowerPoint is a must
Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
Must be willing to work weekends, nights, and long/flexible hours as required by the position.
Up to 75% of travel is required
