Health Home Care Manager Supervisor (Health Home)

Hybrid | New York City
$75,000 – $80,000 + Excellent Benefits

ACCM is seeking a motivated and innovative Care Manager Supervisor to lead our Care Management and Intake teams. We are looking for someone who enjoys solving problems, improving processes, developing staff, and implementing strategies that drive better outcomes for members. If you're a hands-on leader who thrives in a fast-paced environment and wants to make a meaningful difference, we'd love to hear from you.

Key Responsibilities

Lead, coach, and develop Care Managers and Intake Specialists
Improve team performance, member engagement, and operational efficiency
Oversee assessments, care plans, enrollments, and care coordination activities
Support staff with complex member cases and care transitions
Ensure compliance with Health Home and regulatory requirements
Drive quality improvement initiatives and workflow enhancements
Collaborate with leadership to support growth and innovation

Qualifications

Bachelor's degree in Social Work, Human Services, Public Health, Healthcare Administration, Psychology, or related field
2+ years of Care Management, Health Home, Case Management, or Social Services experience
Strong leadership, communication, and organizational skills
Supervisory experience
Health Home experience preferred
Experience with Foothold and Epic preferred
Bilingual Chinese preferred
