JOB TITLE: CCBHC Clinical Pathway Coordinator

REPORTS TO: CCBHC Program Director

DEPARTMENT: CCBHC

LOCATION: ONSITE

The Clinical Pathway Coordinator (CPC) is responsible for overseeing and authorizing the client intake process, from the initial session to the initial treatment plan. We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance with a team at a specified Brooklyn clinic location with some travel in support of the CCBHC Project.

SUMMARY: Clinical Pathway Coordinator (CPC) is a key clinical provider within IDCCs CCBHC program dedicated to integrating mental health, substance use and medical care. The CCBHC Program provides targeted case management, mobile crisis intervention and ongoing treatment in mental health, medical health and substance use across all IDDCC clinics. The CPC ensures that each intake stage is approved promptly while maintaining a continuous check on the quality of the client's documentation. The CPC reviews and ensures the appropriate services are being offered, including but not to limited to the CCBHC core services.

Qualifications: The person must be experienced with the intersection of mental health, substance abuse and physical health needs. Activities include the engagement, referral oversight, facilitation of intake, communication and triage of service needs and ensuring care pathways are appropriate. Must be experienced with transitions in care.

JOB DESCRIPTION:

Ensures that each intake stage is approved promptly while maintaining a continuous check on the quality of the client's documentation.
Sign Checklists: The CCBHC Director will review and sign the checklists for Intake 1, Intake 2, and Intake 3 as they occur for quality and compliance. Each checklist must be signed within 24 hours of its respective intake.
As part of the intake 3, the CPC to go into PSYCKES and update clinical summary and include in recommendations.
Provide Clinical Oversight: After each intake session, the CPC will with Program Directors to review the documentation ensuring clinical integration and clinical decision support aligning with the intake assessment and IDCC approved screening tools. During this review, the CPC will either create or update the pathway tracker form to ensure the clinician is thoroughly documenting and clarifying the needs of the client. This ongoing review is a critical part of the process.
Final Documentation & Service Planning: If the intake is completed prior to intake 3 the CPC will complete the CPC note. On the day third intake or the first session after the completion of the intake (if the intake was completed after Intake 1 or 2) or the first kept session after a brief intake, the CPC will create a Clinical Pathway Coordinator Note indicating the level of care and clinical care pathway assigned to the client. This note documents the client's needs and core services, and ensures they either receive those services or are presented with a clear set of choices.
After intake is completed and signed and the client is admitted in CareLogic to the CCBHC program, CPC will begin the Treatment plan recommendations or family based on the identified needs outlined in the assessment, clinical care pathway highlighting the appropriate Evidenced Based Practices (EPC).
Receive and process referrals to determine appropriate level of care.
The CCBHC Director will provide direct supervision of the CPC. The CCBHC Director will meet with the CPC weekly for individual supervision and oversight.
Ensure receipt of proper referral: Request and track the receipt of supportive documentation of a consumer's mental health treatment, core history, prior hospitalizations, etc.
Review referral for both routine and urgent cases and formulates the appropriate level of care to meet the identified needs of the consumer, particularly for those transitioning from a higher level of care.
Identify the clinical pathways for the coordination of care for consumers with mental illness, substance use disorder, health conditions and barriers to social determinants.
Review and reassess risks, including but not limited to suicide or self-harm, to inform prioritization of assignment for intake.
Collaborate with CCBHC staff, community mental health providers, referral sources and other professionals as necessary to inform eligibility, assessment and assignment.
Monitor a caseload of all high-risk clients and ensure adequate transition to all core services, including clinical pathways (i.e., suicide, diabetes, CVD, obesity).
Review cases on an ongoing basis to reassess for risk to inform prioritization of clinical pathway, assess for violence and provider consultation/technical assistance.
Escalate situations requiring higher level attention and/or intervention.
Triage, review and monitor case management assignment, intakes, outcomes and program effectiveness, making adjustments as necessary to improve client outcomes.
Develop partnerships with community organizations, healthcare providers, and other stakeholders to enhance service delivery and access.
Monitor financial performance and identify opportunities for cost savings and revenue generation.
Ensure compliance with billing and reimbursement requirements, maximizing revenue potential while maintaining program integrity.
Establish and maintain quality assurance processes to monitor and evaluate the effectiveness of services.
Ensure compliance with regulations, accreditation standards, and reporting requirements.
Participate in audits/reviews to ensure adherence to program requirements and standards.
Assists with new program development and program evaluations.
Responsibility to ensure and document partnership/collaboration with community partnerships and collaborations to enhance service delivery and access.
Perform other work-related duties as assigned.
Available to travel and work evening and weekend hours as required by the grant and agency needs.
Work collaboratively with the CCBHC Interdisciplinary Team as well as external agencies and community stakeholders.
Attend all required management and staff meeting and complete all CCBHC and organizational trainings.
Maintains all documentation within agency standards.
Performs duties at the highest ethical standards.

EXPERIENCE:

2 to 4 years of experience with adults with mental health and/or substance abuse needs.
Experience in providing crisis management in a mental health treatment environment.
Experience navigating behavioral health systems and substance treatment systems.
Excellent written and verbal communication
Well organized with strong time management skills
Enthusiasm and self-motivation regarding identifying client needs and obtaining related resources.
Able to work independently and as a member of a team
Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues
Proficient with word processing, electronic health records, and electronic communications.
Excellent written and verbal communication.
Able to meet program standards in documenting in client records.
Ability to work with challenging clients.
Comfortable with community outreach and home visits.
Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.
Bilingual skill in a language other than English valued.

QUALIFICATIONS:

LCSW, LMHC-D or Licensed psychologist.
