Job Description

We are seeking a high-performing Home Care Marketer who knows how to

generate referrals, build census, and produce measurable results.

If you already have relationships with discharge planners, social workers,

MLTCs, or community referral sources—and you know how to convert them

into admissions—we want to speak with you.

This role is not entry-level. Must have demonstrated success in LHCSA

Marketing with ability to independently generate referrals through strong

professional presence and follow-through.

This is for someone who understands the NY home care market and can

deliver results.

Key Responsibilities

 Develop and manage referral relationships with hospitals, rehab

centers, nursing homes, clinics, physicians, social workers, and

community organizations

 Drive consistent patient referrals and increase agency census

 Represent the agency professionally at community events, meetings,

and outreach initiatives

 Maintain ongoing communication with referral sources to ensure

satisfaction and repeat referrals

 Track outreach activity and referral results

 Work closely with intake and operations teams to ensure smooth

admissions

Requirements

 Minimum 2–3 years of home care marketing experience (LHCSA

required)

 Established referral source relationships preferred

 Strong knowledge of Medicaid, MLTCs, and private pay home care

 Knowledge of OPWDD waiver program

 Excellent communication and relationship-building skills

 Self-motivated, organized, and results-driven

 Valid driver’s license and ability to travel locally
