Full Job Description

Company Overview

JCCA is a non-profit agency dedicated to serving children and families since 1822. Our mission is to provide high-quality programs that empower those in need, ensuring they have access to essential resources and support.

As an FTC Facilitator, you’ll lead family-centered conferences that bring together caregivers, service providers, and community supports to create solutions that promote safety, permanency, and well-being. Your work will support the mission of the NYC Administration for Children’s Services (ACS), using a strength-based, collaborative approach.

Key Responsibilities

Facilitate all types of Family Team Conferences, including Service Termination and 30/45-Day conferences
Maintain fidelity to the ACS Family Team Conference Model
Track, schedule, and document all conferences to ensure timeliness and compliance
Conduct pre-conferences, debriefings, and follow-up to support progress on family goals
Build strong partnerships with families, service providers, and community stakeholders
Conduct home visits when needed to hold conferences in the most supportive environment
Maintain accurate documentation in CONNECTIONS, PROMIS, and agency databases
Participate in training, supervision, and staff meetings to uphold model fidelity
Serve as an active interdisciplinary team member supporting the overall success of each case

Qualifications

Bachelor’s degree in Social Work or related field from an accredited institution
Strong facilitation and mediation skills to navigate group dynamics
Excellent communication, writing, and organizational abilities
Proficiency in Microsoft Word and Excel
Knowledge of NYC child welfare principles and practices
Willingness to work a flexible schedule (evenings & at least one Saturday/month)
Ability to travel throughout the NYC area

Preferred Qualifications

At least two (2) years’ experience in child welfare or family support services
Previous training in Family Team Conferencing or group facilitation
Exceptional interpersonal skills and the ability to engage diverse participants in collaborative problem-solving
