As a Registered Nurse (RN) Case Manager, you’ll provide high-quality, compassionate care to patients in the comfort of their homes throughout Schoharie County.

What You’ll Do

As an RN Case Manager, you’ll serve as both caregiver and care coordinator — ensuring patients receive comprehensive, individualized, and compassionate care.

Key Responsibilities:

Conduct admissions, assessments, and physical evaluations for new patients.
Develop personalized care plans in collaboration with physicians and interdisciplinary teams.
Provide skilled nursing services, treatments, and patient education.
Supervise and guide LPNs and Home Health Aides to ensure quality, continuity, and compliance.
Document visits accurately using electronic medical records (EMR) technology.
Communicate regularly with families, caregivers, and the care team to support positive outcomes.
Meet clinical and regulatory standards while maintaining compassionate, patient-centered care.

What You Bring to the Team

Current Registered Nurse (RN) License – active and in good standing with the NYS Department of Education.
Associate’s degree in Nursing required; BSN preferred.
Minimum 1 year of home care or clinical nursing experience.
Solid comfort level with Electronic Medical Record (EMR) technology and digital documentation.
Strong communication, assessment, and problem-solving skills.
A passion for helping others and serving your local community.
