Full Job Description

Job Title: Patient Advocate in a nursing home

Position Summary

The Patient Advocate serves as a key liaison between patients, families, and the facility, ensuring concerns are addressed promptly, rights are upheld, and care experiences are positive. This role focuses on communication, problem-solving, and patient satisfaction while supporting regulatory compliance and facility standards.

Must work every other weekend

Key Responsibilities

Act as the primary point of contact for patient and family concerns, questions, and grievances
Advocate for patient rights, dignity, and informed decision-making
Investigate complaints, document findings, and coordinate resolutions in a timely manner
Collaborate with Admissions, Nursing, Social Work, and clinical teams to resolve issues
Educate patients and families on facility policies, and available resources
Assist patients and families with navigating the internal healthcare system
Escalate issues to respective departments and maintain accurate records of grievances and outcomes
Ensure compliance with New York State Department of Health and federal regulations
Participate in quality improvement initiatives and patient satisfaction efforts
Support surveys, audits, and inspections as needed
Complete required Admission Agreement paperwork with patient / family

Qualifications

Bachelor’s degree preferred (Social Work, Healthcare Administration, Psychology, or related field)
Minimum 1–3 years of experience in healthcare, patient advocacy, social services, or customer relations
Experience in a skilled nursing or sub-acute setting strongly preferred
Knowledge of patient rights, HIPAA, and NYS healthcare regulations
Strong interpersonal, communication, and conflict-resolution skills
Ability to handle sensitive situations with professionalism and empathy
Excellent documentation and organizational skills
Bilingual Spanish required
