Full Job Description

Home Care Agency is seeking a Certified Dietitian/Nutritionist (CDN/RDN) to add to our HRSN Services team. In this role, you will be responsible for conducting comprehensive nutrition assessments, developing medically tailored nutrition plans, and approving meal and food prescription services as we work to become a leading provider of Health-Related Social Needs (HRSN) services within the Medicaid population.

As a Certified Dietitian/Nutritionist, you will help identify nutrition-related health risks, develop individualized nutrition interventions, and ensure compliance with Medicaid 1115 Waiver nutrition service standards. You will be responsible for clinical oversight of medically tailored meals, nutritionally appropriate food prescriptions, and home-delivered meal programs. You will also work collaboratively with interdisciplinary care teams to improve health outcomes, food security, and chronic disease management for Medicaid members.

Duties and Responsibilities:

Conduct comprehensive nutrition assessments for Medicaid members with chronic conditions or special nutritional needs.
Develop individualized nutrition care plans aligned with medical diagnoses and care plans.
Provide nutrition counseling and education, including healthy meal preparation and grocery budgeting guidance.
Approve medically tailored meals in accordance with clinical nutrition guidelines.
Approve clinically appropriate home-delivered meal plans.
Authorize medically tailored or nutritionally appropriate food prescriptions.
Review and approve meal boxes and pantry stocking services to ensure nutritional adequacy.
Ensure all nutrition services comply with Medicaid 1115 Waiver and HRSN program requirements.
Collaborate with care managers, home care staff, and community partners to coordinate services.
Document all assessments, interventions, and approvals in accordance with Medicaid billing and compliance standards.
Participate in conferences, supervision, and quality improvement initiatives.
Maintain current knowledge of evidence-based nutrition practices and Medicaid regulations.

Required Experience/Skills:

Bengali Speaking preferred.
Active New York State Certified Dietitian/Nutritionist (CDN) credential or Registered Dietitian Nutritionist (RDN) certification required.
Bachelor’s degree or higher in Nutrition, Dietetics, or related field required.
Minimum of 2-3 years of clinical nutrition experience preferred.
Experience working with Medicaid populations or underserved communities preferred.
Knowledge of nutrition services under the 1115 Waiver or comparable food security programs preferred.
Experience developing medically tailored meal plans for individuals with chronic conditions.
Strong understanding of food insecurity, chronic disease management, and culturally responsive nutrition care.
Ability to complete accurate and timely electronic documentation.
Strong communication and interdisciplinary collaboration skills.
Ability to travel within the community as needed.

This is a part-time (20-30 hours/week) fully remote.

Job Type: Part-time

Pay: $31.00 - $40.00 per hour

Expected hours: 25.0 – 30.0 per week
