Location: NYC Hybrid (Primary Location: VISIONS, 500 Greenwich Street, New York, NY 10013)
Salary Range: $67,000-$75,000 per year

Summary

VISIONS Citywide Caregiver Program, funded by NYC Aging, provides support to unpaid caregivers caring for an adult age 60+, an adult of any age with dementia or Alzheimer’s, an adult age 18–59 with a disability (when the caregiver is age 55+), or children under 18 cared for by older family members. VISIONS serves caregivers of individuals with vision loss, hearing loss, dementia, and other disabilities across all five boroughs, in multiple languages and accessible formats.

The Caregiver Outreach Coordinator is a key member of the Caregiver Program team, supporting program goals through the development and implementation of outreach, public information, and community engagement activities. The Coordinator is responsible for planning, conducting, and documenting outreach, public education, professional in-service training, and community relations efforts to identify and engage caregivers across New York City, with a focus on underserved populations.

The Outreach Coordinator works collaboratively with VISIONS staff and subcontractors to plan and execute outreach events and ensure that caregivers are informed of available services and successfully connected to the program intake process in accordance with NYC Aging requirements.

Essential Duties and Responsibilities

Meet and maintain NYC Aging public information and outreach service levels, ensuring consistent outreach coverage across all five boroughs and community districts.
Build and maintain strong relationships with community partners, including businesses, social service organizations, housing programs, healthcare providers, faith-based organizations, and other referral sources; identify and engage underserved and hard-to-reach caregiver populations.
Conduct targeted outreach to identify isolated and underserved caregivers and provide information on available services; facilitate connection to the program and ensure referrals are made for intake, with appropriate follow-up and documentation.
Respond to general inquiries regarding VISIONS Caregiver Services and provide accurate program information; ensure outreach and educational materials are current, accessible, and available in multiple languages as needed.
Plan and conduct presentations, workshops, and professional in-service trainings for community-based organizations and service providers to increase awareness of caregiver services and promote referrals.
Ensure alignment with NYC Aging requirements by conducting outreach activities across all community districts and maintaining appropriate geographic distribution of outreach efforts.
Represent VISIONS at health fairs, community events, and public forums to educate caregivers and the broader community on available services and caregiver needs.
Participate in meetings with elected officials, community boards, borough offices, interagency councils, and other public forums, as assigned, to increase program visibility and strengthen referral networks.
Coordinate and collaborate with subcontractors to plan and implement outreach activities and ensure consistent messaging and citywide reach.
Support program visibility through digital and print outreach efforts, including maintaining listings in resource directories and contributing to social media and public information campaigns.
Maintain accurate and timely documentation of all outreach activities in required systems, including VIVE; ensure all outreach events are entered promptly and that supporting documentation (e.g., sign-in sheets, flyers) is collected, filed, and maintained for monitoring and annual assessment purposes.
Track outreach activities, referrals, and engagement outcomes to support program reporting and performance monitoring.
Participate actively in supervision, staff meetings, and interdepartmental outreach coordination meetings.
Perform other duties as assigned to support program operations and outreach goals of the agency.

Education and/or Experience

Bachelor’s degree with relevant experience or Master’s degree in Social Work (BSW/MSW) or a related field (e.g., social services, public administration, public health) required.

Qualifications

Minimum of two years experience in working with older adults, caregivers, and/or individuals with disabilities, with a demonstrated commitment to community engagement.
Strong interpersonal and communication skills, with the ability to effectively engage community members, service providers, and referral partners.
Fluency in English required; proficiency in Spanish, Chinese, or other languages commonly spoken in New York City is preferred.
Ability to travel independently throughout all five boroughs of New York City to conduct outreach, presentations, and community engagement activities.
Strong organizational skills, with the ability to manage multiple outreach activities and meet reporting and documentation requirements.
Ability to work effectively with individuals from diverse cultural, linguistic, and socioeconomic backgrounds.
Proficiency in computer applications, including Microsoft Word, Excel, email, internet-based systems, and data entry; ability to learn and utilize electronic tracking systems such as VIVE.

Skills and Reasoning Ability

Strong attention to detail and ability to manage multiple tasks accurately in a fast-paced environment.
Ability to work independently while also collaborating effectively with program staff, subcontractors, and community partners.
Strong relationship-building skills and ability to interact professionally with a wide range of stakeholders, including clients, vendors, and community organizations.
Demonstrated problem-solving and organizational skills, with the ability to prioritize workload and meet deadlines.
Excellent written and verbal communication skills, including the ability to present information clearly in both individual and group settings.
Understanding of and commitment to the mission and philosophy of a nonprofit social service organization, including sensitivity to the needs of caregivers and individuals with disabilities.

Salary Range

$67,000-$75,000, Commensurate with experience
