Case Manager – Newport Office

Northeast Kingdom Council on Aging

Do you enjoy supporting older adults in creating their own path to aging? Are you passionate about helping people maintain independence, dignity, and quality of life in their own homes? The Northeast Kingdom Council on Aging is seeking a dedicated Case Manager to join our growing team in our Newport office.

As a Case Manager, you will play a vital role in supporting older Vermonters and adults with disabilities who wish to remain safely and independently in their homes and communities. This position focuses on assessment, individualized care planning, coordination of services, and ongoing support, working closely with clients and community partners to create meaningful, integrated care solutions.

Key Responsibilities

Conduct in-home assessments and client visits.
Develop and implement individualized care plans.
Support clients in achieving independence and personal goals.
Coordinate services with community partners and agencies.
Maintain accurate documentation and client records.
Uphold confidentiality and professional standards.

What We’re Looking For

A genuine passion for serving older adults and individuals with disabilities.
Strong interpersonal, written, and verbal communication skills
Ability to collaborate effectively with community partners.
Strong organizational skills, accuracy, and attention to detail
Comfort using computer systems and databases.
Commitment to ethical practice and confidentiality
Ability to work independently in the community (home visiting required)

Qualifications

Bachelor’s degree required.
Valid driver’s license and reliable transportation required.
Experience in human services, social work, aging services, or related field preferred.
