The Maples Health and Rehabilitation is currently looking for an experienced Social Services Manager with experience in a Post Acute Care/Long-term care setting.

The Purpose of the Social Work position:

To provide support to residents and their families in coping with placement in a long-term care facility. Also, to be the understanding, helpful, and caring person they turn to at this difficult time.

The primary duties of this job include:

Work primarily for the Post Acute part of the facility with a census of 25-30
Perform assessment of the resident at admission, upon change of condition and/or annually; create, review and update care plan and progress notes
Responsible for all NOMNC's/ABN's for residents
Assist resident’s family in understanding Post Acute along with Medicare/Insurance benefits
Schedule, and interview patients/representatives to obtain required admission information, including identification records and copies, and signatures on all applicable admission paperwork
Coordinate resident visits with outside services -dental, optical, etc.
Work with the resident, family and outside referral agencies such as home health and hospice in assuring proper placement
Attend and document resident care plan meetings to help document resident progression/goals
Conduct job responsibilities in accordance with the Company’s Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standards and applicable state/federal laws
Coordinate with nursing to schedule admissions, transfers, and discharges of patients as required
Perform additional duties as assigned
Ability to make routine decisions in accordance with departmental policies and procedures.

Experience/Education

The Social Worker should have knowledge of practices applicable to a long-term care environment and be skilled in directing and motivating the staff. Will work cooperatively as a member of a team. Need to communicate effectively with residents, their families and at all levels of the organization and maintain confidentiality.

Prefer Candidate have BSW or degree in related field.

Previous experience in a hospital, skilled nursing care facility, or other related medical facility is required.
