What You'll Do

Provide comprehensive primary care services to patients with multiple chronic comorbidities
Deliver care through a combination of clinic visits and field-based patient interactions
Build strong relationships with patients while improving quality outcomes
Help establish operational processes and clinical workflows
Collaborate closely with interdisciplinary care teams
Contribute to the long-term growth and success of an innovative care model

Required Experience

Nurse Practitioner with a minimum of 5 years of primary care experience
Strong background managing patients with chronic and complex medical conditions
Experience in Internal Medicine, Family Medicine, Geriatrics, Hospital Medicine, or similar primary care-focused settings
