Compensation: $110,000-$210,000 annually + $15,000 Hiring Bonus

Legacy Home Care and Home Health Care, Inc. are expanding our Physical Therapy coverage throughout Southern and Southwestern Minnesota, creating an opportunity for a PT who wants to build a significant career right where they live.

In return, we're offering $110,000-$210,000 in annual earning potential, a $15,000 Hiring Bonus, and a Monday-Friday schedule with no weekends or holidays.

This Isn't a Sit-in-the-Same-Clinic-Every-Day PT Position

This role is built for a therapist who likes movement, variety, and independence.

Your office changes throughout the day.

Your patients change.

The communities you serve change.

You'll travel throughout an assigned portion of Southern or Southwestern Minnesota, bringing Physical Therapy directly to patients who may otherwise have limited access to high-quality home-based rehabilitation.

For the right therapist, the windshield isn't a drawback. It's freedom.

You're not tied to the same treatment room all day. You're trusted to manage your territory, organize your caseload, care for your patients, and communicate with your clinical team.

Become the PT Your Territory Depends On

There's something different about practicing in smaller and rural Minnesota communities.

Healthcare resources can be more limited. Patients may have fewer options. Traveling to outpatient therapy multiple times each week isn't always realistic.

That's why bringing therapy directly into the home matters.

You'll help patients recover following hospitalization, surgery, illness, and injury while addressing the real-world barriers preventing them from safely remaining independent.

Your work may be the difference between a patient continuing to live safely at home and needing a higher level of care.

What You'll Do

Provide in-home Physical Therapy evaluations and treatment visits.
Complete Start of Care assessments and admissions.
Develop individualized plans of care.
Address strength, balance, gait, transfers, mobility, and functional independence.
Educate patients, families, and caregivers.
Identify fall risks and barriers to safely remaining at home.
Supervise and collaborate with Physical Therapist Assistants.
Coordinate care with nurses, physicians, therapists, and other interdisciplinary team members.
Monitor patient progress and modify treatment plans when appropriate.
Complete accurate and timely documentation and OASIS assessments.

Who We're Looking For

We're especially interested in an experienced clinician who is independent, organized, comfortable traveling, and confident managing a regional caseload.

Home health and OASIS experience are preferred, but we're also interested in PTs coming from Acute Care, Outpatient Therapy, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, Orthopedics, and other rehabilitation environments.

Qualifications

Current Minnesota Physical Therapist license.
PT or DPT.
Strong clinical assessment and decision-making abilities.
Ability to independently manage a territory and caseload.
Comfort with regional travel throughout Southern or Southwestern Minnesota.
Valid driver's license and reliable transportation.
Excellent communication and patient education skills.
Home health experience preferred.
OASIS experience preferred.
