Job Summary:

This position is responsible for providing personal care related to the daily living activities of hospice patients. The RN Case Manager (RNCM) is a key component and is responsible for facilitating key relationships within the communities and facilities they serve.

Supervisory Responsibilities:

· Certified Nursing Assistant (CNA)

· Licensed Practical Nurse (LPN)

Duties/Responsibilities:

· Assume primary responsibility for the patient and family needs that includes assessing, planning, implementing and evaluating care needs.

· Maintain communication with providers and staff members to coordinate optimal care and use of resources for the patient, family and facility.

· Provide education regarding medications, side effects, dosing instructions, etc. to the patient, family and facility.

· Assess psychological, social and spiritual needs of the patient and family and notify the appropriate social worker and/or chaplain when warranted to meet the needs of the patient.

· Coordinate and communicate medication changes and orders with the Nurse Practitioner and or hospice physician.

· Coordinate delivery of medication, equipment and supplies with patients, family and facilities.

· Communicate equipment and supply needs with the Director of Nursing.

· Assist in determining the need to adjust the level of care provided based on nursing assessment in collaboration with the Director of Nursing, Hospice Physician, Nurse Practitioner, and attending physician.

· Document all on-call activity including but not limited to, phone calls and visits in the EHR within 24 hours.

· Document all routine and PRN visits, phone calls, and patient interactions in the EHR within 72 hours.

· Evaluate and update patient care plans and/or care plan problems every 14 days.

· Provide appropriate guidance at the time of death including but not limited to, witness medication disposal, notification to county coroner, funeral home and postmortem care.

· Implement care to achieve outcomes of a comfortable and safe death.

· Conduct monthly supervisory visits with LPN and Hospice Aides.

· Maintain communication with facilities and caregivers through the hospice binder, calendar and visit record note entries each visit.

· Abe to demonstrate the competency to complete the initial comprehensive assessment upon hospice admission.

· Ability to assist with and operate patients with physical and mechanical transfers.

· Ability to properly stage a wound and obtain treatment plan from provider. Communicating the same plan to family, caregiver and facilities as appropriate.

Required Skills/Abilities:

· Knowledge of appropriate HIPAA and confidentiality

· Maintain accurate documentation

· Able to work as both a part of a team and independently

· Possesses great organizational Skills

· Excellent time management skills.

· Excellent written and verbal communication skills

· Valid driver’s license and auto insurance

· Ability to use EHR systems and operate mobile devices with efficiency

· Knowledge of Microsoft products i.e. Excel, Word, etc.

Education and Experience:

· Graduate of Accredited Nursing Program: minimum associate degree (AND); bachelor’s degree (BSN) preferred

· Posses a valid Minnesota Registered Nurse (RN) License

· Current CPR/BLS Certification

· Minimum of 1 year nursing experience in healthcare setting

· One year hospice or long term care experience preferred

· Willingness to obtain the certification in Hospice and Palliative Care within 2 years of employment.

Physical Requirements:

· Able to regularly lift and move up to 50lbs

· Regularly operate and assist with physical and mechanical transfers

· May have prolonged periods of sitting at a desk and working on a computer

· Personal protective equipment provided, including masks
