Key Responsibilities

Overseeing Clientele that is assigned to your care
Creating and maintaining relationships with our Caregivers and Nurses caring for your client
Executing, implementing, and updating the Plan of Care for the client, ensuring the Caregiver is aware of changes
Home visits - 75% of your time will be spent out in the field specifically in the Weymouth, Quincy, and Braintree areas
Onboarding prospect clients
Monitoring Discharge from Hospital, SNF, or Rehab
Completing administrative paperwork in accordance with our policy and NYS DOH

Qualifications

MA Registered Nurse with Up to Date License
Ability to work autonomously
Ability to work some weekends throughout the month
Must feel comfortable clinically training RNs and Caregivers on-site to their respective duties for their clients
