Full Job Description

Job Title: Lead Case Manager – HHS-LA

Reports to: Program Manager

Status: Full-Time, Non-Exempt

Location: Santa Fe Springs

SUMMARY

The Lead Case Manager for Housing and Health Services (HHS) – Los Angeles is responsible for providing case management for housing stability and supportive services aimed at improving health outcomes, wellness, and recovery for members. This role requires strong leadership to ensure the provision of high-quality case management services and documentation to vulnerable individuals needing support to address barriers that may impact their stable housing and health. This position serves to provide support to the team in the absence of the Program Manager. Case Management services are expected to be provided at field and home settings on a regular basis, with documentation completed within twenty-four hours of delivery of service.

ESSENTIAL DUTIES & RESPONSIBILITIES

1. This position requires on-site presence.

2. Work directly with the leadership team to implement program goals.

3. Ensure that services to members are reflective of AFH’s model, which emphasizes customer choice, harm reduction, and psycho-social rehabilitation.

4. Provide case management services and referral services to all eligible members in areas including but not limited to housing stability support, job training and linkage, education, financial literacy and management, income development, acquisition of benefits, medical and mental health support, and substance use support.

5. Support members with daily tasks, emergencies and crisis situations.

6. Conduct outreach and screening for potential members.

7. Collaborate with case managers to ensure member documentation and data collection are accurate, complete, and current in all agency records and electronic databases through regular audits and chart reviews.

8. Maintain up-to-date service records in Electronic Health Record systems.

9. Provide input, evaluations, and updates as needed to support program improvement.

10. Maintain confidentiality of members’ files per HIPAA and agency policies and follow all AFH Policies and Procedures as outlined by HUD, HSH, HCA, LAHSA, HA’s contracts or any other contracts.

11. Participate in program and community partner meetings to provide updates and program progress.

12. Coordinate with agency partners to provide service coordination for mental health and medical services for program participants.

13. Support the Program Manager’s efforts in team development through open communication, support to staff and participation in meetings and trainings.

14. Provide accompaniment and guidance to case managers when addressing challenging or escalated member situations to ensure safety and service quality.

15. Serve as point of contact for case managers by answering questions and offering support as needed.

16. Assist with developing PowerPoint presentations and training materials for onboarding and staff development.

17. Carry a reduced caseload to allow time for leadership and support duties.

18. Serve as team lead in the absence of the Program Manager.

QUALIFICATIONS

1. Bachelor’s degree in social sciences (Social Work, Psychology, Human Services) or related field or 2 years’ experience working with homeless populations and/or individuals at-risk for homelessness.

2. Knowledge and experience in chemical dependency and substance abuse treatment preferred.

3. Current, valid California Driver’s License with an acceptable driving record and reliable vehicle.

4. Able to transport clients in your vehicle.

5. Detail oriented with excellent time management, organizational, communication, interpersonal, and computer skills.

6. Able to work in a Harm Reduction model with clients who have multiple barriers.

7. Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.

8. Self-starter with the ability to stay ahead of the curve and thrive in fast-paced environment.

9. The ability to work well under deadlines and to multitask.

10. Proficient skills in Microsoft Office, including Word and Excel.

11. Excellent verbal and written communication skills, and critical thinking and problem-solving skills.

12. Able to obtain and maintain CPR/1st Aid Certification.

WORK ENVIRONMENT

1. Field and indoor office environment.

2. Will necessitate working in busy and loud environments.

3. Will be exposed to elements like cold, heat, dust, noise, and odor.

4. May need to bend, stoop, twist, and sit throughout the day.

5. Regularly required to walk or drive to different local sites throughout the day.

6. Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds.

Pay: $25.00 - $26.75 per hour
