About the Role:

We are seeking a highly motivated and compassionate Social Services Designee to join our team at The Avenue at Westshore, a premier new skilled nursing facility in Holland, Michigan. As a Social Services Designee, you will play a vital role in ensuring the well-being and quality of life for our residents. If you are passionate about delivering exceptional care and making a difference in the lives of others, we encourage you to apply.

Responsibilities:

Assess the social, emotional, and psychological needs of residents and develop personalized care plans to address these needs
Provide guidance and support to residents and families to ensure smooth transitions and resolutions to social and emotional concerns
Facilitate communication between residents, families, and healthcare professionals to ensure effective care coordination
Develop and implement socialization programs to promote resident engagement, wellness, and overall quality of life
Collaborate with interdisciplinary teams to identify and address social and emotional issues affecting residents
Document resident progress and maintain accurate records of social services provided
