Employment Duration: 2 months (w/ potential for extension)
The LVN utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio-economic background, and response to medical regimen and treatment.
In collaboration with health care providers, nurses, and medical assistants, the LVN will assume clinical responsibility for administration of medications/vaccines, lab result review and monitoring, treatments and procedures, as well as patient education.
This individual will utilize good customer service and communication skills daily via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers. This employee will utilize computer skills in ApeX and other UCSF computerized programs.
This individual supports the standards of the policies and procedures of the practice and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration. They comply with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. They are required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrate UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.
Key Responsibilities:
Assessment
- Identifies the needs and responses of patients to medical regimen and treatment through observation of symptoms, emotional state, and cultural-socioeconomic backgrounds.
- Recognizes and reports specific physical and emotional conditions, needs, and concerns of the patient to appropriate members of the health care team. Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g., chest pain and shortness of breath.
- Learns how to respond in a code blue scenario and understands use of emergency equipment.
Reports emergency or unsafe patient care conditions on the Incident Reporting System and to the management team.

- Works cooperatively and participates with the health care teamlet to discuss patients’ alternatives to health care services and to develop short- and long-range goals to meet patients’ needs.
- Plans as appropriate for the needs of patients using knowledge of physical and emotional processes as well as cultural and socioeconomic backgrounds.
Identifies appropriate resources, including family, significant others, monetary resources, community organizations, and online or written teaching tools, in planning for patient care.

Implementation and Evaluation
- Provides individualized nursing care that responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient.
- Demonstrates ability to provide appropriate care based on the needs of a specific individual, including the individual’s age.
- Sets up and maintains cleanliness of exam rooms, including periodic exam room disinfection and organization and removal of expired supplies.
- Performs the intake process and engages in inreach and outreach to support practice quality initiatives and the model of population management. Provides for patients’ physical and emotional privacy.
- Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments.
- Assists as needed with diagnostic and therapeutic procedures and treatments, including vaccines, injections, wound care, and phlebotomy.
- Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC on CLIA-waived tests and reports any tests out of QA range to the supervisor.
- Uses appliances and equipment correctly and maintains safe practice.
- Evaluates and documents patients’ responses to nursing care in the electronic medical record, including pre/post procedures and treatments. Uses knowledge of cognitive, physical, emotional, and chronological maturation processes in evaluating care for all ages of patients.
- Administers and documents the administration of medication while maintaining current knowledge of pharmacology and competency in modes of administration.
- Documents telephone messages and chart notes in a thorough and grammatically correct manner.
- Responds appropriately in emergencies. Is knowledgeable about the contents of the code blue cart and/or emergency kits and can initiate and assist in CPR. Understands procedures for medical emergencies, code blue, and other emergency/disaster situations. Participates in safety drills.
- Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. Complies with activities mandated by TJC/Title 22/NCQA.
Completes annual competency testing as specified by the individual practice.

Coordination, Continuity, and Communication
- Organizes activities based on the needs of patients while maintaining the effective flow of patients through the practice. Adjusts clinical support activities to reflect changes in priorities during practice sessions.
- Maintains open communication and facilitates continuity of care for patients and/or families with members of the health care team through phone contact and verbal interaction.
- Maintains accurate records and documentation, including patient complaints, nursing observations, interventions and care plans, nursing actions/teaching performed, and patient responses to the action.
- Obtains, records, and conveys information clearly and concisely.
- Demonstrates courtesy, common sense, and follow-through in all telephone/electronic communications, maintaining the Telephone Standards established by the Medical Center.
- Completes all tasks within assigned InBasket, including cross-coverage of InBaskets.
- Fosters and maintains positive public relations with patients, families, visitors, and staff. Uses effective communication skills with patients and staff; demonstrates proper telephone techniques and etiquette; shows sensitivity to differences of culture; and demonstrates a positive and supportive manner.
- Demonstrates ability to work as a team member with providers, supervisors, fellow nursing staff, and clerical/administrative staff, with the practice team, and in cross-organization relationships.
- Participates in daily huddles to support efficient and patient-centered care.
