Full Job Description

SNAP Case Manager

DEPARTMENT: Non-Residential

LOCATION: Branan Center

SUPERVISOR: Clinical Supervisor

STATUS: Non-Exempt

CLASSIFICATION: Professional

SUMMARY:

Responsible for completing intakes, facilitating groups with children and families, and collaborating with the local schools to provide weekly groups. Providing transportation if needed to all family members participating in the weekly groups.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide Snap services that include an assessment of the problem, development of service plans to address needs identified, linking families with appropriate services, monitoring the case plan or progress, and advocating for the youth and family.

Meeting face-to-face with the family and youth to complete all required assessments and paperwork.

Complete weekly phone calls to family to assist with any barriers to attendance and to determine transportation and sibling supervision needs.

Complete a Fidelity Adherence Monitoring Checklist after each group session. The checklist will be entered into Netmis within three (3) days.

Videotaping of each group is completed during groups and uploaded to Dropbox within three (3) business days.

Serve as liaison with the Department of Juvenile Justice, Department of Children and Families, and other agencies, concerning the needs of youth and their families served by Arnette House.

Prepare and submit required reports, statistical data, and correspondence as required.

Maintain documentation in the charts.

Provide transportation for children and families as needed.

Be available for on-call.

Participate in outreach activities as needed.

Provide in-service training for staff as needed.

Participate in staff meetings, clinical staffing, and supervision as required.

Perform other duties as assigned.

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION/EXPERIENCE/CERTIFICATION:

This position requires a minimum of a Bachelor’s Degree in a related field of study, and two years of experience working with adolescents and families. A verifiable background demonstrating responsibility for character and ethical conduct is required. The individual must also possess a valid Florida driver’s license and carry the minimum automobile insurance required by Florida law, and have access to reliable transportation.

MATHEMATICAL SKILLS:

Ability to perform basic mathematical functions in all units of measure and graph numerical and

statistical data.

REASONING AND COMMUNICATION ABILITY:

Exhibit a positive role model for staff, clients, and members of the community. Ability to solve practical problems and deal effectively with a variety of situations. Ability to communicate effectively with employees, clients, and members of the community. Ability to develop and interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to work with culturally diverse staff and clients. Must be self-motivated to do tasks as needed.

stoop, kneel or crouch. The employee may occasionally lift and/or move more than 25 pounds. Specific vision abilities include close vision, peripheral vision, depth perception, and the ability to adjust focus.

While performing the duties of this job, the employee frequently works near other co-workers and may be required to share workspace.

The noise in the work environment is usually moderate. There is the potential to interact with various size groups of troubled youth ages 8-17 years of age. While this position does not require formal supervision of the clients, as a role model to those clients receiving services.
