ACO Care Coordinator – Transitions of Care

Location: Remote (U.S.) or In-Person (Florida)
Employment Type: Full-Time
Compensation: Competitive hourly wage based on experience

Help Patients Stay Healthy at Home

Our physician-led Medicare Accountable Care Organization (ACO) is seeking an organized, compassionate, and self-motivated Care Coordinator to help patients successfully transition from hospitals, emergency departments, skilled nursing facilities, rehabilitation centers, assisted living communities, and other care settings back to their primary care provider.

This role is about building relationships, solving problems, and making sure patients never fall through the cracks. Every successful transition helps reduce unnecessary emergency department visits, hospital readmissions, and improves patients' quality of life.

This position may be performed remotely within the United States or in person at one of our Florida locations.

Healthcare experience is preferred; however, we are willing to train an exceptional candidate who demonstrates outstanding communication skills, organization, initiative, and a genuine passion for helping patients.

What You Will Do

· Monitor hospital admissions and discharges for ACO patients.

· Build relationships with hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, home health agencies, and physician offices.

· Contact patients and caregivers after discharge and coordinate timely follow-up appointments.

· Coordinate services including home health, specialists, laboratory testing, imaging, therapy, durable medical equipment, and community resources.

· Identify barriers that could lead to emergency department visits or hospital readmissions.

· Follow each patient until outstanding issues are resolved and the transition is complete.

· Maintain accurate documentation in the electronic health record and care-management system.

· Escalate urgent clinical concerns promptly to the licensed clinical team.

Qualifications

Experience in one or more of the following is preferred:

· Care coordination

· Transitional Care Management (TCM)

· Case management

· Hospital discharge planning

· Population health

· Home health

· Skilled nursing

· Assisted living

· Medicare or Medicare Advantage

· Accountable Care Organizations (ACO)

Preferred professional backgrounds:

· Licensed Practical Nurse (LPN)

· Registered Nurse (RN)

· Care Coordinator

· Case Manager

· Hospital Care Navigator

· Social Worker (BSW or MSW)

· Community Health Worker

· Patient Navigator

· Discharge Planner

· Population Health Coordinator

· Utilization Review or Care Management Specialist
