Full Job Description

Job Summary

We are seeking passionate and dedicated Bilingual Licensed Clinical Social Workers (LCSWs) fluent in Russian to join our team. In this role, you will identify and address social, behavioral, and economic barriers impacting patient health outcomes and access to care. Responsibilities include providing clinical counseling, psychosocial assessments, crisis intervention, care coordination, and connecting patients with community resources and support services. The LCSW will collaborate with patients, caregivers, providers, health plans, and community organizations to improve care coordination, promote health equity, support population health initiatives, and enhance overall patient well-being in a culturally sensitive environment.

Duties

Conduct comprehensive psychosocial and Social Determinants of Health (SDOH) assessments for assigned patients.
Identify barriers related to housing instability, food insecurity, transportation, financial hardship, utility assistance, medication affordability, social isolation, and access to healthcare services.
Develop individualized patient-centered care plans addressing identified social and behavioral needs.
Coordinate referrals to community-based organizations, social service agencies, behavioral health providers, home care services, and support programs.
Provide ongoing care coordination and follow-up to ensure patients successfully connect with recommended resources and services.
Assist patients with applications and enrollment for Medicaid, SNAP benefits, HEAP, transportation assistance, housing resources, and other government or community programs.
Collaborate with physicians, nurse practitioners, nurses, care coordinators, and health plan representatives to support integrated patient care.
Participate in interdisciplinary care team meetings, case conferences, and utilization management discussions.
Support transitions of care following hospital discharges, emergency room visits, rehabilitation stays, or skilled nursing facility admissions.
Educate patients and caregivers regarding available community resources, healthcare navigation, and self-management strategies.
Conduct outreach calls and patient engagement activities to assess ongoing social needs and barriers to care compliance.
Assist with closing preventive care and quality gaps by addressing non-clinical barriers impacting patient compliance.
Advocate for patients and families to ensure appropriate access to healthcare, and community support services.
Maintain current knowledge of local, state, and federal social service programs and community resources.
Document all patient interactions, interventions, referrals, and outcomes accurately and timely within the electronic medical record and care management systems.
Ensure documentation and workflows remain compliant with organizational policies, CMS, Medicaid, Medicare, NCQA, and other regulatory requirements.
Identify patients requiring escalation to behavioral health services, crisis intervention, adult protective services, or additional clinical support.
Support population health and health equity initiatives focused on improving outcomes for vulnerable and underserved populations.
Track and report patient outcomes, referral completion rates, and barriers impacting care management goals.
Participate in quality improvement initiatives, audits, training sessions, and departmental meetings.
Assist with patient education related to chronic disease management, preventive screenings, medication adherence, and community wellness programs.
Build and maintain collaborative relationships with community organizations, healthcare providers, and social service agencies.
Maintain confidentiality and protect patient information in accordance with HIPAA regulations and organizational policies.
Perform additional duties and special projects as assigned.

Qualifications

Valid licensure as a Licensed Clinical Social Worker (LCSW) in New York State
Fluency in Russian and English is essential for effective communication with clients from diverse backgrounds.
Proven experience in motivational interviewing, child & family counseling, and working with children or individuals with disabilities.
Knowledge of medical terminology, mental health diagnoses (ICD-10/ICD-9 coding), CPT coding, and medical documentation standards.
Familiarity with EMR systems and experience in data collection or clinical research is preferred.
Strong skills in crisis management, case management, patient assessment, and behavioral health interventions.
Experience working in hospital settings or home & community care environments is advantageous.
Ability to handle sensitive situations involving PTSD care, addiction counseling, autism spectrum disorders, Geriatrics, or Alzheimer's care with professionalism and compassion.
