Physical Therapist
JOB SUMMARY
Organize and conducts medically prescribed physical therapy programs to restore function and prevent
disabilities following a disease, injury, or loss of a body part. Help patient reach His/her maximum
performance while learning to live within the limits of His/her capabilities.
SUMMARY OF ESSENTIAL JOB FUNCTIONS
● Provides care for the Adult or Geriatric patient, ages 18 years and over.
● Demonstrates and understands the importance of and respect for the rights, dignity and
individuality of each patient in all interactions.
● Performs Physical Therapy evaluation(s) as prescribed by physician and establish therapy
programs for each patient, involving physical means, such as exercise, mobility training, massage,
heat, water, and electricity, as indicated by analysis of evaluations.
● Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests.
● Delegates responsibilities and supervise activities of physical therapy assistants while maintaining
knowledge of the patient’s status and revise patient’s care plan as warranted.
● Directs and aids in patients inactive and active assistive exercises, muscle reeducation, and gait
and functional training, utilizing pulleys and weights, steps, and inclined surfaces.
● Directs patient in care and use of wheelchairs, braces, assistive devices, and prosthetic and
orthotic devices.
● Gives instructions in posture control and therapeutic exercises to be continued by the patient.
● Adapts conventional physical therapy techniques to meet the needs of patients with a variety of
communicative and/or physical deficits.
● Submits accurate patient charges in a timely manner.
● Evaluates, records, and reports on patient's progress for review by other members of the
rehabilitative team.
● Documents short and long term goals within an appropriate time frame and update as needed.
● Participates in discharge planning and patient and family education in order to establish a safe
discharge environment.
● Conducts or participates in training medical and nursing personnel and other workers in physical
therapy techniques and objectives.
● Develops an understanding of responsibilities for participation and participate in Performance
Improvement activities.
● Educates / Instructs patients and families in safe transfers, ambulation and appropriate therapeutic
exercises.
● Attends scheduled staff meetings, departmental meetings, in-services and training programs.
● Actively participate in continuing education programs for self-development.
● Performs other duties as assigned or delegated by the Manager.

STANDARD JOB DUTIES
● Adheres to all components of the Hospital Compliance Plan in performing job duties and reports
any violations or suspected violations of the Plan to the Compliance Officer.
● Demonstrates professional conduct and complies with hospital and departmental policies and
procedures.
● Adheres to hospital attendance policy as outlined in the Employee Handbook.
● Complies with measures for preventing exposure to blood borne pathogens.
● Participates in the Hazardous Material Program; hazards include exposure to blood and body
fluids, possible communicable diseases, sharp objects and instruments, assorted chemicals and
gasses as listed in the Hazardous Materials Program Manual.
● Complies with established Safety and Patient Safety Program practices.
● Performs other incidental and related duties, as required and/or assigned.
● Develops an understanding of responsibilities for recognizing patient abuse and follows policy for
making appropriate referrals/interventions.
● Develops an understanding of responsibilities for participation in Performance Improvement
activities and participates in such activities.
● Limits access to protected health information (PHI) to the information reasonably necessary to do
the job, and shares such information only on a need-to-know basis for work purposes.

Education/Experience:
Bachelor’s degree required, Master’s preferred. Current PT license to practice by the licensing board in the
state of employment..
Computer Skills:
Knowledge of basic computer skills and word processing.
Certificates and Licenses:
Current PT license to practice in state of employment. Must obtain BLS within 30 days of employment and
maintain for the duration of employment.
