Full Job Description

Non-profit agency seeking self-motivated Case Manager to provide housing-focused services to homeless men, women, and families toward successfully transitioning out of shelter into permanent housing.

RESPONSIBILITIES

Work closely with partner agencies and landlords to facilitate case coordination and successful transitions into permanent housing.
Refer and direct clients to appropriate resources within the community.
Complete housing inspections, required paperwork, and lease agreements.
Handle case assignments, develop individual client goal plans, review progress and determine case closure.
Enter data into the Utah Homeless Management System (ClientTrack).
Document and write comprehensive case notes
Adhere to professional standards, code of conduct, and protocols

QUALIFICATIONS

Excellent computer skills (Microsoft Office & Microsoft excel)
Ability to work in a team environment
Effective communication skills both written and verbal
Excellent organizational and time management skills
Ability to problem-solve, multi-task, prioritize and work under pressure
Bachelor’s Degree in Social Work or related field

Job Type: Full-time

Pay: $17.00 - $19.00 per hour

Expected hours: 40 per week
