Full Job Description

CLD3 Counseling is a licensed facility through the Department of Health and Human Services, Office of Licensing to provide outpatient mental health, domestic violence, and substance treatment services. We have served clients throughout the State of Utah since 2008. Our mission is to provide a safe and secure environment for clients to engage in the process of change. We strive to work with statewide agencies to assist clients through their change process.

We are currently seeking an LCSW/CSW or LCMHC/ACMHC to work primarily with domestic violence offenders, assist with group therapy, and work with families and children. The position involves completion of thorough mental health, domestic violence, and substance evaluations. The work week will include 20 to 30 hours of weekly sessions, with the potential for full-time hours (32+) within a couple of months of hire.

JOB REQUIREMENTS

Master's Degree
LCSW/CSW or LCMHC/ACMHC
Domestic Violence Certification (we provide certification training, if needed)
Clinical experience with diagnostic formation, treatment planning, progress summary, and discharge planning
Community collaboration and frequent communication
Basic computer and electronic health record system skills
Competence with document writing
Completion of psychometric assessments and questionnaires, including incorporation into documents and charting
Collaborate with team members

JOB DUTIES

Completion of mental health, domestic violence, and substance evaluations
Maintaining timely and thorough documentation
Creation of treatment plans, progress summaries, and discharge plans
Maintaining quarterly treatment plan reviews
Individual, Couple, Family and Group Sessions
Compliance with Division of Child and Family Services contractual duties
Compliance with Medicaid Provider contractual duties
Completion of licensure requirements, including continuing education and licensure standards
Maintain active certification with Columbia Suicide Severity Rating Scale (C-SSRS), Campbell Danger Assessment, and Domestic Violence Certification
Compliance with the Office of Licensing's Code of Conduct
Positive engagement with team members
Supervision with our Clinical Director, Clinical Supervisor, and Administrative Director to assist with skills development, professional growth and expansion, and to meet licensure requirements.
