Position Title: NCW Program Coordinator (Nursing Home Transition, Case Manager-Supervisor for Roads To Independence (RTI) New Choice Waiver program)
Short Description: As the NCW Program Coordinator at RTI, you'll play a key role in supervising 3 to 5 staff that transitioning individuals with disabilities out of nursing homes and supporting them in setting and achieving their independent living goals and maintaining their independence in the community.
Agency Description: Roads To Independence (RTI) is a non-profit, grassroots, advocacy organization by and for people with disabilities. Core services include but are not limited to Advocacy, Information and Referral, Life Skills Training, Peer Support, Recreation, Assistive Technology and Nursing Home Transition and Diversion. We primarily cover Davis, Morgan, and Weber Counties but serve people in Salt Lake and Box Elder counties on the New Choices waiver as well. The New Choices waiver is a Medicaid program that assists individuals to live independently in the community rather than in a nursing home.
Status: Full Time
Minimum qualifications require applicant to be a licensed Social Worker (SSW, CSW, LCSW, BSWA, or CSWI) or Register Nurse.
Salary: Based on experience (Range 32 to 36 an hour/ $66,560 to $74,880), based on experience with 8% Retirement Benefits, Health Insurance, Dental, Health Savings Account, Paid Vacation and Sick leave, and other benefits. People with disabilities encouraged to apply.
Reports to: Day to Day direction by Program Manager and Executive Director. Working in collaboration with our Registered Nurse, Independent Living Specialists and other staff.

The NCW Program Coordinator oversees Roads To Independence’s New Choices Waiver (NCW) program and assists people with disabilities in transitioning out of nursing homes or other institutional settings and back into the community and their own homes. Individuals served are often newly injured or have age-related disabilities and require coordinated attendant services and supports to achieve successful community living.
This position provides program coordination, staff supervision, compliance oversight, and direct services to a limited caseload of consumers. The Coordinator develops and oversees individualized plans of care and Independent Living Plans (ILPs) and coordinates services such as attendant services, nursing, skills training, emergency backup planning, benefits management, assistive technology, self-advocacy, housing, and other NCW services and supports.
The NCW Program Coordinator ensures services align with the Independent Living philosophy while meeting required state documentation, reporting, and audit standards.
Essential Duties and Responsibilities
Program Coordination & Compliance
- Oversee day-to-day operations of RTI’s NCW program to ensure compliance with state, federal, and contractual requirements.
- Ensure accurate and timely submission of all required NCW reports, audits, case documentation, billing support documentation, and data requests.
- Serve as the primary point of contact for NCW-related coordination with state partners, nursing facilities, and community providers.
- Monitor program performance, service utilization, and outcomes using spreadsheets, tracking tools, and reports (Excel proficiency required).
- Prepare for and participate in audits, quality assurance reviews, and corrective action planning as needed.
Supervision & Leadership
- Supervise, support, and evaluate NCW Transition Coordinators/Case Managers (3–4 staff).
- Provide coaching, training, and technical assistance related to documentation standards, care planning, Independent Living philosophy, and compliance.
- Support HR-related functions including onboarding, performance reviews, corrective action, and professional development in coordination with leadership.
- Promote accountability, teamwork, and a consumer-centered service culture.
Direct Services (Limited Caseload)
- Maintain a caseload of approximately 8–10 NCW consumers.
- Develop individualized Independent Living Plans (ILPs) and care plans, including goals, timelines, and services to be provided.
- Assist consumers with successful transitions from nursing homes or institutions into the community.
- Coordinate services such as attendant services, nursing, skills training, emergency backup plans, benefits management, assistive technology, housing, and self-advocacy.
- Identify and address barriers to community integration and independence.
Advocacy & Community Engagement
- Advocate for issues that promote independence and full inclusion of people with disabilities.
- Provide or coordinate independent living skills training.
- Disseminate information and conduct presentations in nursing homes and community settings.
- Develop and maintain public-private partnerships to support successful transitions.
- Develop and implement outreach and marketing strategies to increase awareness of programs and services.
General Duties
- Attend designated meetings, trainings, and general center meetings.
- Travel regularly throughout the service area to meet with consumers and partners.
- Perform other duties as assigned.
Qualifications
Required
- Licensed Social Worker (SSW, CSW, LCSW, BSWA, or CSWI).
- Supervisory or leadership experience, including staff oversight and performance management.
- Strong organizational and time-management skills with the ability to prioritize multiple responsibilities.
- Demonstrated experience with documentation, reporting, and audit compliance.
- Proficiency in Microsoft Excel and other data-tracking tools.
- Excellent written and verbal communication skills.
- Ability to work independently and exercise sound judgment.
- Reliable transportation and ability to travel within a multi-county service area.
- Successful completion of a Criminal Background Check.
Preferred
- Experience working with individuals with disabilities.
- Knowledge of Independent Living philosophy and consumer-controlled services.
- Familiarity with PEGA, nursing home transition programs, and HCBS waivers.
- Knowledge of medical terminology and its impact on independent living.
- B ilingual abilities.
- Personal experience with disability.
Working Conditions: Work schedule requires flexibility and is dependent upon program needs. Work is typically performed in an office environmentally controlled setting. The position requires traveling out of area and working flexible hours, occasionally including week ends and e venings.
