Full Job Description

Position Summary

Moonlight Care Services LLC is seeking an experienced Quality Assurance Coordinator with a strong background in Ohio’s developmental disabilities (DD) service system.

This is not a general healthcare or corporate quality assurance position.* The ideal candidate will have direct professional experience working with individuals with intellectual and developmental disabilities and a working knowledge of Ohio Department of Developmental Disabilities (DODD) requirements, County Boards of Developmental Disabilities, Medicaid waiver services, MUI/UI processes, provider compliance, and residential or community-based DD services*.

The Quality Assurance Coordinator will support agency-wide quality assurance, compliance, training, documentation, and continuous improvement efforts. This individual will work closely with agency leadership, direct support teams, SSAs, County Boards, guardians, and other members of individuals’ interdisciplinary teams to ensure services are safe, person-centered, compliant, and consistent with each individual’s ISP and applicable regulatory requirements.

This position reports directly to the Quality Assurance Director and requires an individual who is comfortable working independently, collaborating across departments, identifying areas of concern, and following through to ensure corrective actions are completed.

Key Responsibilities

Conduct routine quality assurance and compliance reviews across residential and community-based developmental disability services.
Review service documentation to ensure services provided are consistent with individuals’ ISPs, authorized services, agency policies, Medicaid requirements, and applicable DODD standards.
Conduct routine reviews of homes and service locations to identify documentation, health and safety, medication, staffing, training, and service-delivery concerns.
Review UI and MUI documentation for completeness, accuracy, timeliness, appropriate follow-up, and corrective action.
Coordinate with leadership regarding MUI/UI follow-up, prevention plans, corrective actions, and recurring trends.
Monitor agency documentation systems and ensure DSPs and administrative staff maintain timely, accurate, and compliant records.
Identify patterns, deficiencies, and systemic concerns through audits and develop practical recommendations for improvement.
Follow up on identified deficiencies to ensure corrective actions are completed rather than simply documenting concerns.
Assist with internal investigations, compliance reviews, and preparation of supporting documentation when requested.
Provide training and technical guidance to DSPs, house managers, coordinators, and other staff regarding documentation, individual-specific requirements, incident reporting, individual rights, health and safety, and regulatory expectations.
Provide thorough compliance and documentation training to newly hired employees following completion of HR onboarding.
Participate in ISP meetings, County Board meetings, SSA meetings, provider meetings, and interdisciplinary team discussions as needed.
Maintain internal quality assurance tools, audit schedules, corrective-action tracking systems, and compliance checklists.
Assist the Quality Assurance Director with DODD reviews, County Board reviews, accreditation activities, Medicaid/provider compliance, and audit preparedness.
Collaborate with Operations and other departments to ensure quality expectations translate into actual service delivery.
Provide leadership with regular feedback regarding recurring compliance concerns, staff training needs, operational risks, and opportunities for improvement.
Support accreditation, licensure, certification, and regulatory preparedness as needed.
Promote a culture of accountability, person-centered services, individual rights, dignity, choice, safety, and continuous quality improvement.
Remain current with changes in DODD requirements, agency policies, and applicable regulatory standards.

Required Qualifications

Bachelor’s degree in Human Services, Social Work, Healthcare Administration, Psychology, Public Health, Business Administration, or a related field preferred.
Minimum of 5 years of professional experience in the developmental disabilities field, community-based services, residential services, or a closely related human-services environment.
Previous experience in quality assurance, compliance, program management, residential management, service coordination, provider operations, or another management/leadership-level DD role strongly preferred.
Working knowledge of the Ohio DODD service system and regulatory environment strongly preferred.
Knowledge of MUI/UI reporting, prevention, documentation, and follow-up requirements.
Familiarity with ISPs, Medicaid waiver services, County Boards of Developmental Disabilities, SSAs, DSP requirements, individual rights, incident reporting, and provider documentation expectations.
Demonstrated ability to critically review documentation and identify compliance or service-delivery concerns.
Strong written and verbal communication skills.
Ability to train, coach, and provide constructive feedback to employees.
Strong organizational skills with the ability to manage multiple responsibilities, deadlines, and follow-up activities.
Ability to communicate professionally with individuals served, families, guardians, SSAs, County Board personnel, DSPs, and agency leadership.
Must be comfortable visiting residential and service locations and addressing concerns directly and professionally.
Must have reliable transportation and the ability to travel locally to agency service locations.
Flexibility is required when urgent compliance, incident, or service concerns require attention outside of the normal schedule.
Preferred but not required: Current Ohio professional licensure or certification in a related behavioral health or social services discipline, including LSW, LISW, LISW-S, LPC, LPCC, LPCC-S, or another relevant professional credential.
