Full Job Description

Summary

Perform professional consultation, and educational services for persons with diabetes, and provide community health education. Develop curriculum on diabetes management and act as a health care consultant by providing information on the management of diabetes for patients in our Western Hills and Harrison Ohio location .

Clinical Duties

· Promote the mission, vision, and values of Crossroad Health Center

· Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; blood glucose monitoring and techniques of self-monitoring and injection

· Provide patient-centered education for other patients with diagnoses including, but not limited to hypertension, prediabetes and obesity.

· Support Crossroad Health Center’s quality goals through proper documentation of clinical activities.

· Maintain a schedule of patient appointments while also providing education for patients who are unscheduled, or seeing another provider.

· Develop and implement educational programs for diabetic patients and families

· Assist patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and non-insulin dependent diabetes

· Provide ongoing assessment and recommendations to the treatment term and patient/family for adjustment in the plan of care as required

· Document assessments, interventions and plans including recommendations, goals and progress notes

· Assist in the development of policies and procedures for diabetes and health education guidelines

· Train and educate nursing staff in diabetes and health clinical procedures; coordinate in-service training, continuing education, and other related diabetes clinical training

· Provide care management for patients who qualify for the care management program at CHC by identifying patients who meet the qualifications and may benefit from participating in the program, developing care plans with the patient including patient centered goals, creating and updating care plans in the EMR, following up with patients at in person visit and by phone, and updating care plans.

Knowledge, Experience and Skills

. Muat have RN Licensed .

· Certified Diabetes Educator

· Must maintain certification as a Certified Diabetes Educator

· Able and willing to assist with training of volunteers that may be needed.

· Good organizational skills. Able to coordinate multiple tasks well.

· Good communication skills, verbal and written.

· Willingness to live and promote Crossroad’s mission.

Job Type: Part-time

Pay: From $29.00 per hour

Expected hours: 16 – 24 per week
