Full Job Description

Reentry Targeted Case Manager

Okanogan County Juvenile and Family Services

The Reentry Targeted Care Manager is a licensed Registered Nurse responsible for coordinating healthcare and reentry services for Medicaid-eligible individuals transitioning from incarceration to the community. This role combines clinical expertise with care management to ensure timely Apple Health enrollment, individualized care planning, and access to medical, behavioral health, and Health-Related Social Needs (HRSN) services. The Case Manager provides nursing oversight, ensures compliance with federal and state requirements, and collaborates with detention staff, Managed Care Organizations (MCOs), and community providers to promote safe and successful reentry outcomes.

This position is funded through a grant supporting the HCA Reentry Initiative. Funding is guaranteed for two years, with continuation dependent on future grant availability. The grant provides stable support for the duration of the project and offers a unique opportunity to advance innovative reentry and healthcare coordination efforts.

This is a full time non union/non exempt position with full benefits including: Medical, dental, vision, basic life and long term disability insurance. Employer/employee retirement with options and deferred compensation options. Paid annual and sick leave accruals at 8 hours per month each to start. Eleven paid holidays and 2 floating holidays.

Qualifications

EDUCATION/TRAINING 1. Current, active Registered Nurse (RN) license in Washington State required. 2. Bachelor of Science in Nursing (BSN) preferred. EXPERIENCE 1. Minimum of three (3) years of nursing experience, including care coordination or case management. 2. Prior experience in correctional health, public health, or behavioral health settings preferred. 3. Experience with justice-involved youth or adults strongly preferred. 4. At least one (1) year of supervisory or oversight experience desirable.

Special Requirements

CONDITIONAL EMPLOYMENT REQUIREMENTS 1. Successful completion and maintenance of First Aid and CPR certification. 2. Maintenance of a valid Washington State driver’s license. 3. Successful background check, including criminal and child abuse records. 4. Successful drug testing, polygraph, and psychological evaluation.
